To Provide Mental Health Counseling and Case Management Services to Underserved Low Income Individuals and Families in Sheridan, WY. In 2024, the organization carried out its exempt purpose by providing accessible, community-based behavioral and mental health services to children, adolescents, adults, and geriatric populations experiencing emotional, psychological, and psychiatric challenges. Services were delivered in outpatient, community, and telehealth settings to reduce barriers to care and improve continuity of treatment. Throughout the year, the organization provided comprehensive behavioral health assessments, individualized treatment planning, evidence-based psychotherapy, skills-based interventions, crisis intervention, care coordination, and case management. Clinical services addressed conditions including depressive disorders, anxiety disorders, post-traumatic stress disorder (PTSD), attention-deficit/hyperactivity disorder (ADHD), bipolar disorders, trauma-related disorders, adjustment disorders, and co-occurring substance use concerns. Services incorporated cognitive behavioral therapy (CBT), trauma-informed care, solution-focused interventions, family systems approaches, and psychoeducation to support symptom stabilization, improved functioning, and relapse prevention. In 2024, the organization served approximately 72 unduplicated individuals and provided over 1783 clinical service encounters. A significant percentage of clients were low-income, uninsured, underinsured, or Medicaid beneficiaries. Services were delivered without discrimination and included sliding fee scale accommodations and coordination with schools, primary care providers, probation systems, and community partners to ensure continuity of care. Outcomes achieved during the year included measurable reductions in symptom severity, decreased psychiatric hospitalizations and crisis episodes among high-risk individuals, improved school attendance and academic engagement for youth clients, increased employment stability among adult clients, improved medication adherence through coordinated psychiatric support, and strengthened family functioning through family therapy services. The organization further advanced its exempt purpose by providing mental health education, crisis response support, and referrals to community resources addressing social determinants of health such as housing instability, food insecurity, and transportation barriers. These efforts contributed to improved overall well-being and enhanced community stability. Through these activities, the organization fulfilled its charitable and exempt purpose of promoting mental health, reducing the burden of mental illness, and improving access to quality behavioral health care for vulnerable and underserved populations in 2024.$0$36,098
