zero8hundred proactively links transitioning military families, recent veterans, Reservists, National Guard and/or their spouses to the broad range of resources & services in the community, helping them more successfully transition to civilian life.

| For most of 2018, the organization was a fiscally sponsored program of Mission Edge San Diego. In July of 2018, the organization was established as an independent public charity. Through the organization's agreement with Mission Edge, Zero8Hundred focused on the following programs: Military/Recent Veteran Transition Support: zero8hundred coordinates outreach to recruit program participants before they leave the military by conducting presentations about the zero8hundred program for attendees at their mandatory on-base transition classes. The presentation is typically delivered to the entire class and includes general information about transition opportunities and challenges. Service members/spouses who plan/or would like to remain in the southern California region, after separation from the military, are encouraged to enroll in the program through the online portal at www.zero8hundred.org/enrollhere. Additionally, the program details and opportunity to enroll are provided at a number of transitioning spouse classes and Yellow Ribbon Reintegration workshops around the county as well as other military command and community-sponsored events. zero8hundred?s 3-tiered model caters to varying levels of need among zero8hundred program participants. Tier I helps participants, who already have a robust and achievable transition plan, to identify and connect the vetted community programs and services who could best support them in fully executing their plan and gaining the self-sufficiency necessary to successfully transition to our civilian communities. These program participants are paired with a Resource & Outreach Specialist intern, under the supervision of a full-time staff clinician. Tier II offers individualized assistance to identify transition goals, obstacles and opportunities and helps program participants to craft and implement their transition plan with peer navigation and counseling. These program participants are paired with more seasoned Resource & Outreach Specialist interns, under the supervision of a full-time clinician. Extensive transition services are offered in Tier III for transitioning personnel who have multiple and or complicated barriers to transition, including the impact of PTSD, Military Sexual Trauma and traumatic brain injury, with these services delivered only by full-time Resource & Outreach Specialist clinicians. There is no fee structure for the zero8hundred program. All services, regardless of level of need, are provided at no cost to program participants. Resource & Outreach Specialists provide direct case management support through telephone, email, and in-person meetings. Further, these Resource & Outreach Specialists conduct follow-up communications to ensure all aspects of the program participant?s transition plan are executed appropriately, recording all outcomes for reporting and program improvement. Resource & Outreach services include (1) referrals to job placement, (2) providing advice on educational opportunities, (3) providing connections to health and wellness services, including information regarding physical and mental health screening/diagnostic services, and (4) providing access to basic needs resources and peer connections. All of zero8hundred?s services form part of its holistic wellness model, a model designed to take into account all of a transitioning service member/spouse?s specific needs and not just those that might be immediate or most critical at the time of enrollment. Service and resource referrals are derived from community information exchanges (2-1-1 San Diego, 2-1-1 Orange County, and 2-1-1 Community Connect Riverside and Imperial counties) ? comprehensive and fully vetted databases of existing social service agencies and employment opportunities in those communities. This project supports program participants who plan to live, work, or go to school in San Diego, Riverside, Imperial or Orange counties after separation from active duty. Additionally, it serves those veterans and their spouses, who separated only within the past 12 months and are living, working, or going to school in one of these four counties. To date, over 3,400 program participants have enrolled to receive transition support with approximately 1,000 receiving peer counseling and resource navigation support at any given point in time. | - | - |
