The 990 is reviewed by the chief executive officer. The complete form 990 is provided to the finance and executive committee as well as as to the board for their review prior to filing with the internal revenue service.

Form 990, Part VI, Section B, Line 12C

The organization requires each board member and key employee to complete an annual certification of ethical conduct, conflict of interest and compensation policies. The certification form requires the board members and key employees to indicate if they have any conflicts or have any interest that may give rise to a conflict. The conflict of interest policy is monitored throughout the year.

Form 990, Part VI, Section B, Line 15

The compensation for ceo and other officers and key employees is established based on other similar sized organizations in the area. There will also be a formal process in place to establish on-going compensation. Compensation will be based on looking at similar size and mission organizations in the area as well as measuring performance against agreed upon measures. This will be approved by the board as part of the budgeting process.

Form 990, Part VI, Section C, Line 19

The organization makes its governing documents, conflict of interest policy, and financial statements available to the public upon request.
