Once the return is prepared by the organizations tax advisor or accountant, a draft copy is distributed via email to the full Board of Directors and key officers for their review. Since we filed an extension, the draft of Form 990 was reviewed by the CEO, Patience Rose, the CFO who has financial oversight responsibilities in 2025. This review occurred during the final week of preparation, prior to the return being filed with the IRS.”

Annual Disclosure: All board members and officers are required to complete and sign an annual conflict of interest disclosure statement, affirming their understanding of the policy and disclosing any potential conflicts. 2. Review by Governance Committee or Executive Leadership: Completed disclosure forms are reviewed by the Executive Director and Treasurer, and any identified conflicts are discussed to determine whether recusal or other action is needed. 3. If a conflict exists, the affected individual is asked to recuse themselves from discussion and voting on the matter. 4.
