The executive committee can act on behalf of the corporation in between regular meetings of the board. The board then must ratify, amend, or rescind the actions of the executive committee at its next regular or special meeting.

The form 990 is reviewed by the chief financial officer and the chief executive officer prior to submitting to cla. The governing body, chief program officer and chief administrative officer will review prior to submission to the irs.

Through annual conflict of interest questionnaire to staff and board members. All new hires receive upon hire. Additionally, through lci's conflict of interest policy, staff and board members are required to disclose any conflicts of interests that arise during the year. An audit is completed to ensure that all board and employees have completed the questionnaire.

1. Increase guidelines a. Executive salary evaluations shall occur bi-annually. B. An independent third-party that is recognized to have the requisite background, experience, and qualifications to provide an unbiased report to the board of directors and the ceo, at least biannually. Upon completion of such review by an independent third party and receipt of the third-party report, if any, the board of directors will approve the compensation of the ceo. The ceo will approve the compensation of the cao, cfo, and cpo. 2. Effective date of executive salary increase a. The effective date of the executive salary increase is the pay period corresponding to the first pay date of july. The process described here was last completed in 2024.

LAKELAND CARE, INC. PROVIDES THE SERVICE OF CARE MANAGEMENT TO ENROLLED MEMBERS OF THE PROGRAM.
