Mission

The mission of the GCACH is to advance the health of our population by decreasing health disparities, improving efficiency of health care delivery, and empowering individuals and communities through collaboration, innovation, and community engagement.

Greater Columbia Accountable Community of Health (GCACH) is a collaboration of community leaders from nine counties and the Yakama Nation in Eastern and Central Washington representing a variety of sectors with a common interest in improving population health. By addressing the social determinants, access to care, and care coordination, the organization is driven to reduce duplication of services, identify gaps in the system, improve care coordination, and implement evidenced based programs and services that address priority health issues. The Board of Directors represent 17 sectors related to health and meeting on a monthly basis to guide the work of the organization. The Leadership Council is a broader base of subject matter experts and interested community members who contribute to the various committees and project teams, organized around specific health issues.

IRS990/Desc0During the year ended January 31, 2017, the Organization was designated an Accountable Community of Health (ACH) by the Health Care Authority (HCA). The State of Washington's Health Care Authority, the agency overseeing all nine statewide ACHs, has developed the following key functions of an ACH: 1. Select and monitor Medicaid Demonstration Transformation projects and ensure that the projects are successful2. Serve as the single point of accountability for transformation projects3. Be a defined legal entity in order meet the requirements of the Centers for Medicare and Medicaid Services (CMS)4. Have a governance structure that is multi-sector, has balanced engagement between health care systems and social determinants, is transparent, and demonstrates effective decision making processes5. Demonstrate Regional Partnerships through Letters of UnderstandingGuided by yearly contracts with the HCA, ACHs were required to develop and implement a regional health improvement plan during 2016, and implement at least one regional health improvement project as a pilot project. REGIONAL HEALTH IMPROVEMENT PLAN & PILOT PROJECT:GCACH developed a regional health improvement plan in the Spring of 2016. The plan included a set of coordinated, multi-sector activities that identified and focused on five regional health priorities: 1. Care Coordination 2. Obesity/Diabetes 3. Oral Health 4. Behavioral Health 5. Healthy Youth and Equitable CommunitiesHCA's required that a regional health assessment be conducted across the nine county region, including a needs inventory, strategies to mobilize and coordinate ACH member and partner organizations, an initial action plan, and regional measures to evaluate the effectiveness of the plan. The plan had to propose a project that would address a fundamental issue discovered during the planning process, and use the strategies developed by the community to address it. GCACH chose a hospital readmissions project, called the Readmissions Avoidance Project (RAP) due to the high rates of hospital readmissions occurring in the Tri-Cities Community. The purpose of the project was to coordinate the medical and social services of high risk patients to avoid readmitting them to the hospital within 30 days of their last hospital stay. Among the state's hospital regions, the Tri-Cities had the second highest rate in the state. GCACH contracted with Consistent Care Services, SPC, who enrolled 80 patients into the program utilizing WSU Nursing Students, Meals on Wheels, and Community Health Workers for follow-up care and services. The project was a big success, and dramatically reduced the need for readmissions.COMMUNICATIONS & OUTREACHDuring 2016, GCACH established regular communication channels through a publically facing website, and a monthly newsletter. The Executive Director gave many presentations about the Medicaid Demonstration and progress of the GCACH.MEDICAID WAIVERIn October, the State received word that it had been granted a global 1115 Medicaid Waiver. The Waiver is a state-federal partnership that will bolster the state's efforts to improve the physical and mental health of Washington families and transform the State's Apple Health (Medicaid) program to control costs. The goals of the Medicaid Waiver are: 1. Reduce avoidable use of intensive services and settings such as acute care hospitals, nursing facilities, psychiatric hospitals, traditional long term services and support and jails. 2. Improve population health, with a focus on prevention and management of diabetes, cardiovascular disease, pediatric obesity, smoking, mental illness, substance use disorders and oral health; that is coordinated and whole-person centered. 3. Accelerate the transition to value-based payment, while ensuring that access to specialty and community services outside the Indian Health system are maintained for WA's tribal members 4. Ensure that Medicaid per-capita cost growth is two percentage points below national trends.The Sta
