The trinity wellness foundation is organized and operated to support the activities of trinity behavioral care. We will partner with trinity behavioral care to help people in our communities, regardless of status or income, to expand prevention efforts and improve access to quality substance abuse disorder treatments and other counseling services. Form 990, part vi, line 11b the review process the form 990 is reviewed in detail by the chief exective officer. Then a copy of the final form 990 is sent to all board members, via email for their review, prior to filing with the irs.

Each covered person shall disclose to the chief executive officer, or chairman of the board, as appropriate, all conflicts of interest (1) existing when he or she is offered or elected to a covered position (2) occurring after the acceptance of the covered position and (3) annually through a conflict of interest survey that is distributed by the chief executive officer.the chief executive officer shall report all potential conflicts of interest to the board of directors.
