CARE COORDINATORS ASSIST PATIENTS WITH ONGOING ENROLLMENT IN HEALTH COVERAGE VIA EDUCATION AND NAVIGATION SERVICES, FACILITATING POST DISCHARGE CLINIC VISITS TO HELP REDUCE HOSPTIAL READMISSIONS, AND ASSIST WITH THE EVALUATION OF SOCIAL DETERMINANTS OF HEALTH IN THE PATIENT POPULATION TO CONNECT PATIENTS WITH RESOURCES TO ADDRESS THEIR NEEDS. |
