Significant program services not listed on prior year return Part III line 2

We have 2 seperate projects. 1. The ascend pipeline. We are subcontracted to explore the barriers to accessing medical care after hours and on weekends for north hartford residents. This includes focus groups with hartford residents as well as indvidual interviews with medical providers. We analyzed data and developed a report to guide next steps.2. We are the local evaluators for the transforming communities initiative. This is a 5 year project and last year was year 2. This job entails guiding the data collection,data management and data analysis for the initiative. The purpose of this work is to impact policy,systems and environmental change within north hartford. In addition we provided technical assistance to increase professional capacity.finally, our responsibilities include the creation of reports and presentations for diverse stakeholders including funders, community representatives and elected officials.
