Governance Explanations

Form 990, Part VI, Section B, Line 11B

Form 990 is prepared by DEAP administrative staff. Form 990 is reviewed by the Executive Director. A copy is provided to the Board of Directors for review and acceptance prior to filing the form with the IRS.

Form 990, Part VI, Section B, Line 12C

Annually each board member signs an updated conflict of interest statement identifying any potential conflict of interest issues. These statements are carefully reviewed by Management, Executive Director and Board President. At each board meeting members are asked to declare potential conflict of interest that may be on the agenda. That member(s) is excused from any discussion or actions on that topic. Training is provided annually to board members, management and staff reviewing the agency's conflict of interest policy.

Form 990, Part VI, Section C, Line 19

Form 990 is provided to the public, funders, and others upon request by contacting DEAP's Executive Director or Board of Directors President.

IRS990/Desc0Supported Living (0208 Medicaid Waiver) services are aimed at building independent living skills to adults with developmental disabilities living in their own home or with their family across southeastern Montana. The result of these services are individuals who are actively contributing to their communities and relying on less services over time. DEAP considers serving all individuals with 0208 Medicaid Waiver funding that choose DEAP as their service provider. This program is supplemented with fundraising efforts.
