Each board member is required to read and sign the conflict of interest policy. If the governing board or committee has reasonable cause to believe a member has failed to disclose actual or possible conflicts of interest, it shall inform the member of the basis for such belief and afford the member an opportunity to explain the alleged failure to disclose. If, after hearing the member's response and after making further investigation as warranted by the circumstances, the governing board or committee determines the member has failed to disclose an actual or possible conflict of interest, it shall take appropriate disciplinary and corrective action.

Members of the board led by the board chair set annual goals for the president / ceo aligned with the annual budget, and at the end of the budget year reviews performance against those goals along with industry compensation data provided by the cfao to determine compensation adjustments, if any, for the president / ceo. For other executive compensation, including the cfao, the finance committee reviews as part of the annual budget that the committee presents to the board for approval. The board approves a budget every year that includes detailed compensation for every employee. The board also retained a third party compensation expert to inform the board of appropriate compensation structure and amounts and used this information to determine president / ceo and cfao compensation.

Form 990, Part III, Line 4A, Description of Program Service

The amity foundation provides an array of services based on the therapeutic community model in a variety of physical settings. In general, the services provided for amity participants contain individual and group therapies that are specialized according to the actual setting and recipient of the activities. The amity communities at this time are involved in the following settings: * residential, transition and continuance services provide living quarters as well as the daily therapeutic community model in which participants learn to live as a community, as well as receiving therapeutic individual services, family reunification services, educational and vocational training. * in-prison services provide the therapeutic community model to qualifying members within particular prison settings so they can work on individual and group issues while still incarcerated. This service is particularly effective in prison and has assisted "lifers" (inmates that do not have the option to leave prison), in cleaning up and giving back to their fellow inmates what they have learned. * outreach, community based and prevention services provide therapeutic community model on a daily basis for participants without them living in a residential setting together. Besides receiving individual and group therapeutic services, family reunification, parenting, educational training, vocational training and housing services are provided as well. Services are provided for the substance abuse community and also for hiv/aids at risk populations. * family reunification and parenting services are provided within the therapeutic community model whether it is residential, or outreach/community based. These services are particularly important to forestall the generational patterns of substance abuse and other behaviors related to it. * curriculum training and education are provided to amity staff and individual counselors interested in learning the therapeutic community model. Cross-training has been made available for individuals who provide ancillary services to the amity community in order for them to fulfill their services in a way that is keeping with the therapeutic community model. *we provide case management and other substance use disorder supportive services such as our specialized treatment for optimized programming, and just in reach, which is largely navigation and placement into systems of care. We also serve as a coordinating agency for placement, case management and navigation into other subcontracted substance use disorder services. We provide housing, temporary housing, housing support services and rental assistance, care coordination, community health worker, and employment services such as access, placement and retention strategies for employment, connections to training opportunities, subsidized wages and other employment supportive services for those who have been impacted by addiction, trauma, incarceration, poverty, and homelessness. Finally, amity was selected to act as third-party administrator (tpa) through los angeles county ceo office to manage and distribute a portion of funds included in the cfci spending plan, and non-cfci funds for grants to justice-focused community-based organizations. Amity is responsible for distributing $55 million over three years across eight program areas with the goal of equitably increasing access to funds for organizations serving youth and those impacted by injustice and inequities within the criminal justice systems.

Amounts held for others consist of amounts received and held by the organization on behalf of students participating in their programs. These funds are included in restricted cash and cash equivalents on the statement of financial position.

PART X, LINE 2:

Fin 48 (asc 740) financial statement footnote: the organization is exempt from income taxes under both federal internal revenue code section 501(c)(3) and arizona income tax laws, and is classified as other than a private foundation under section 509(a)(2). Income from certain activities not directly related to the entity's tax-exempt purpose, however, may be subject to taxation as unrelated business taxable income (ubti). Project 5th, project 68th, project 67th, residences and veterans are single-member limited liability companies and, as such, are disregarded entities for tax purposes. Arv is taxable as a corporation; however, there is currently no activity in the corporation so no tax provision has been recorded. From time to time, the organization may be subject to penalties and interest assessed by various taxing authorities, which will be classified as general and administration expenses, if they occur.
