Mission and Program Overview

Mission

To help individuals and families of our SW Iowa communities live independent lives with dignity we will assess needs and develop programs. In 2020 we directed assets to our taxi service (Faith in Action Taxi), our health equipment loan program (H.E.L.P.) and to low or no cost spay/neuter/vaccination assistance for pets, ferals and free roaming community cats (TVNR).

In Fremont County and Southwest Iowa, Faith in Action's projects are developed to support community where residents may live independently with dignity. In 2024 we upgraded our TAXI SERVICE operations with dispatching software. With more drivers and vehicles, our ride count was significantly higher.

We have 3 employees with administrative duties. For each program--TAXI, TNR, HELP/ASSISTANCE--we have advisors to confer with. Administration makes the final decisions with input from those advisors and from the community members who use or benefit from the programs.

Form 990, Part VI, Section B, Line 11B

The program director and volunteer data consultant work up the 990 or 990EZ filing over a period of weeks. We go over it and the supporting documents many times during preparations. We're not against hiring a Non-profit tax preparer, have looked but not found anyone local for the job. We're probably in over our heads and decided after struggling with this year's return, to widen our search.

Form 990, Part VI, Section C, Line 18

Select pages of the 990 will be posted to our website with a link to request more information. The community website Non-Profits of Fremont Co. IA summarizes our 990 each year. We will present select pages of the 2024 990 to Sidney, IA City Council and share copies with select advisors. We will post a link on our website and will have copies available in our office on the town square.

Form 990, Part VI, Section C, Line 19

Our office is open to the public 5 days each week and by appointment. Files are kept on premises and in the office computer. The Program Director's laptop is linked with the office computer. We have staff meetings monthly for training updates and discussion. Quarterly meetings are attended by advisors, volunteers involved with particular projects, donors. Conflict of interest policies from SWIPCO and from the NEMT transportation brokers A2C and MTM are reviewed and renewed annually. We post the policies in our office and put laminated copies in the door pockets of our Taxi vehicles. We keep track of our finances with Quickbooks linked to two banks. A variety of reports are printed and shared at meetings or by request. Excel spreadsheets for each program are kept to track donations and grants both in and out. Cash donations as well as volunteer hours and donated mileage are logged for each program.

The expense breakdown for our three charitable programs are: TNR expenses $7,078. H.E.L.P expenses $679. Fuel for transportation fleet $$23,156 Repair and maintenance for transportation fleet $13,200. Total: $44,113

Form 990, Part XI, Line 9

In 2024 we removed two vehicles from our Taxi Fleet. The 2006 Chevrolet Uplander van @ 198,000 miles, with a persistent electrical problem was retired. We donated it to the Humane Society, they sent it to auction where it brought $1,000 for the Humane Society, not for us. Our 2000 Subaru Legacy Outback was no longer needed for the Taxi Fleet. We made it inactive on the Fleet Vehicle roster and no longer use it to transport passengers. The car needs mechanical repairs, has not been sold. Valued as is: $2,500. Our vehicle asset total was reduced by $4,537. Our two remaining owned Taxi Fleet vehicles in 2024, the 2009 Pontiac Torrent ($1,681)and 2016 Ford Transit Connect ($2,767) remain active in the fleet. Total value of remaining owned assets @ end of 2024: $12,531

IRS990/Desc0Taxi Service by Faith in Action: To deliver community service at the level needed, we contract with Access 2 Care as Non Emergency Medical Transportation Provider. Southwest Iowa Planning Council the Regional Transit Authority provided 3 wheelchair accessible vans for our use to transport our NEMT rides for Medicaid members and for the public. SWIPCO paid registration and insurance for these 3 vans valued @ $9825. SWIPCO and the Area Office on Aging subsidized the non-Medicaid rides we provided @ $5317. FIA submitted monthly van log reports to SWIPCO to document each vehicle's activity including miles, hours, total rides and rides eligible for the subsidy. FIA paid the fuel costs and maintenance and shared the repair costs for all Taxi vehicles. With the wheelchair vans plus our two owned SUVs, we donated rides to the public for a value of $19,751, calculated at $2.16 per mile, the A2C rate. Required by Access to Care our Medicaid Transportation Broker, in 2024 we began the transition to all digital record keeping. We purchased software and hired a tech-savvy young man to implement the new system. Our long-time volunteer with a data processing background worked with him for 6 months to incorporate all aspects of our operation: 3,888 hours. The same volunteer helped set-up Quick Books with the Chart of Accounts reflecting the required 990 information. 1,080 hours including Quick Books set-up and with the initial data entry on the 2024 990. 4,968 volunteer hours valued at $27 per hour equals $134,136. We have not found a local accountant or 990 tax prep pro with availability and are still looking.

IRS990/MissionDesc0In Fremont County and Southwest Iowa, Faith in Action's programs are developed to support a community where residents may live independently with dignity. In 2024 we focused resources on our Taxi Service. With a deadline to transition record keeping to digital, we bought and learned to apply Dispatching Software in the office and on driver smart phones. We had more drivers and vehicles in 2024 to drive NEMT rides for Medicaid members and the public. With the Software in play by June 2024, tracking donated rides became more efficient. In 2024 we fulfilled the purpose of our Charitable Organization by providing an accessible transportation option for our Community.

In 2024 with input from Fremont and Page County community members and the skilled help of volunteers, we transported 64 cats to Hearts United for Animals and other low-cost S/N clinics. 120 volunteer hours @ $27= $3240. 2277 miles were logged @ $1.00 to transport the cats = $2,277. Surgery cost @ $50 each = $3,200. Some cats needed vet care, vaccinations or medications. Pet owners and colony caregivers benefited from our support with donations of food and other supplies valued @ $3,878 for the year. Donations from the public to our TNR program = $3,566.

In 2024 we donated medical equipment valued @ $535 including 2 hospital beds complete, a transport chair and other items. We took in rental fees and 3 donations of equipment all valued at $617. Expanding our financial and services contributions to the Community, in 2024 we spent $679. One person needed car repairs, gas cards for another helped make some long drives to see a specialist, we bought a small fridge for a woman to store her insulin when her refrigerator suddenly died. We filled a community need for non-Medicaid, self pay transportation for two people on Dialysis 3/wk and another requiring rides to/from work: $4,633. We're called on most weeks to provide rides for local errands for shopping, banking, post office, veterinarian: $4,511.

IRS990ScheduleA/FactsAndCircumstancesTestTxt02024 was all about transition. Since 2006, we've had a contract with Access 2 Care, Medicaid transportation broker. Our NEMT (non-emergency medical transport) credentials are renewed annually. We knew early on if we were to develop a level of rural transportation the people wanted/needed, we'd need help. Reaching out to SWIPCO/SWITA the regional transit authority, we agreed on how we'd work together. They "leased" us our first SWITA sedan which we used for County based rides. We pay with monthly log reports of our trip data in 4 counties of SW Iowa. The mutually beneficial partnership has grown steadily with no end in sight. In 2023 A2C issued a mid-2024 deadline for all Providers to transition record keeping to digital. We hired a tech-savvy young man, bought the SOFTWARE, involved our volunteer data manager/consultant to work with the new guy. By the end of 2024, our drivers were confidently logging rides in real time on their smart phones. We have a new Data Manager and now operate our Taxi Service more efficiently. With profits from the business and donations in 2024, we had more funds to support our charitable interests. We've kept an office/meeting place on the Sidney, IA, Fremont County Courthouse Square for years. The Auto Shop next door maintains our taxi fleet. City Hall is at the end of the block. We're in the ideal location for what we do. By 2023 we had established our primary income source with the taxi service. The community remained committed to the TNR, but had less of a need for the medical equipment. We adjusted the programs and committed to go digital for the NEMT program. The required software transition took up much of our time in 2024. Going forward we're positioned to deliver more rides for Medicaid members and the general public. Challenges in 2024 and into early 2025 have been considerable. Most of our time was spent on the Taxi Service operations transition to Dispatching Software and fulfilling the credentialing requirements during the merger of A2C and MTM, Medicaid transportation brokers. We've been understaffed in the office to navigate these changes while carrying on with day to day operations of the NEMT and public transportation services. We used the Taxi Service and tracking software in 2024 to better record the donated rides for our charitable mission. Growing the Taxi Business stretched our limits to handle accounting details. We track finances in house with Quick Books and contract out the Payroll deposits and taxes. We've always done our own 990's. We've reached out to a local Accountancy Firm to take us on as a new Non-Profit client.

SWIPCO paid registration and isurance for these vans valued @ $9825. SWIPCO and the Area Office on Aging subsidized the non-Medicaid rides for ages 60 and over valued @ $5317. FIA submitted monthly van log reports to SWIPCO to document each vehicle's activity including miles, hours, numbers of rides and numbers of rides eligible for the subsidy.
