| ReasonableCauseExplanation/ExplanationTxt | 0 | I was ill due to heart, COVID, and Septic Kidney Disease that caused me to have multiple hospitalizations at Orlando Health and the Washington DC Veterans Administration Health Center in fall 2022 and though Dec 2023. In these cases, I had multiple surgeries and inpatient hospitalizations for these issues that impacted my ability to work. As an office with one full time employee-ME, I was unable to complete this and several other tasks on time. These illnesses took place between Fall 2022 and December 2023. This impacted my filing of the tax paperwork for these years during this time. I can provide necessary documentation of these illnesses should this be necessary. As you can see from our return, we are a program with a very low income. The payment that is required would have an impact on our ability to deliver program. I humbly request that you forgive this debt knowing that I am doing my absolute best to get everything up to date. Your understanding of this matter is greatly appreciated. |
