“The Lighthouse for the Blind of Houston (LOH), a related organization reported on Schedule R, provides management services for Lighthouse Living Centers, Inc. and receives a management fee of 5% of gross collections.”

“The directors elected each year must be the same persons who are directors of LOH. If any LOH director is removed and/or replaced, then Living Centers No. 1 must remove/replace that director on its board as well.”

“Form 990 is reviewed by the finance committee of the Lighthouse for the Blind of Houston. The finance committee prepares a report for the full board of directors. The full board receives the report and Form 990 prior to filing with the IRS.”

“The board members fill out conflict of interest forms annually. These forms are reviewed by the President of the Lighthouse for the Blind of Houston.”

Daniel Williams spent 40 hours per week in his position as VP of Operations for the Lighthouse for the Blind of Houston (through 10/23). He received no compensation for services rendered to Lighthouse Living Centers, Inc. Terry Vaughn, Jr. spent 40 hours per week in his position as VP of Operations for the Lighthouse for the Blind of Houston (as of 11/23). He received no compensation for services rendered to Lighthouse Living Centers, Inc. Chelean Zander spent 40 hours per week in her position as VP of Programs for the Lighthouse for the Blind of Houston. She received no compensation for services rendered to Lighthouse Living Centers, Inc.”

“The Lighthouse for the Blind of Houston (LOH) employs on-site managers and maintenance staff who work for Lighthouse Living Centers, Inc. Living Centers reimburses LOH for salaries, payroll taxes, health insurance and other payroll benefits. LOH files the required W-2s for these employees.”
