Quality trust is an independent catalyst for change in the lives of people of all ages with disabilities in the district of columbia and beyond. We partner with people and their families so they can succeed, thrive and experience full membership in the communities they choose. We complete our mission through a variety of tools including individual and family advocacy, monitoring, legal education, and community outreach.

Governance Explanations

Form 990, Part VI, Section B, Line 11B

A draft copy of the federal form 990 is sent to the investment finance committee of the board of directors for review. The investment finance committee is responsible for assisting management to complete the document consistent with the audit. Investment finance committee meetings are open to any board member who wishes to participate. Once completed, the document is signed and sent to the internal revenue service. Copies of the completed and filed federal form 990 are forwarded to the full board of directors for review and discussion at the next board meeting.

Form 990, Part VI, Section B, Line 12C

Members of quality trust's board of directors complete a conflict-of-interest form at the beginning of their board service and when a conflict arises, or their member status has changed. Staff complete a conflict-of-interest form annually. Any identified conflicts are immediately reviewed and discussed. If the board of directors or chief executive officer (for staff/volunteers) has reason to believe that an interested party has failed to disclose a potential conflict of interest, it shall inform the person of the basis for such belief and allow the person an opportunity to explain the alleged failure to disclose. If the board of directors or chief executive officer (for staff/volunteers) decides that the interested party has in fact failed to disclose a possible conflict of interest, the board of directors shall take such disciplinary and corrective action as the board of directors shall determine.

Form 990, Part VI, Section B, Line 15A

Quality trust salaries are benchmarked to local salary data surveys from local nonprofit and district government agencies. The board of directors initially approved salary ranges for each position and each year the board of directors reviews overall compensation and benefits as part of the annual budget process. Individual performance raises are made within the established salary range. Cost of living adjustments are made as funds allow and with the approval of the board of directors as part of the annual budget process. The board of directors is responsible for establishing the salary and benefits for the chief executive officer. This survey was last conducted during september 2021.

Quality trust provides an annual report to the public which includes the audited financial statements of the previous year. The settlement agreement and consent order establishing quality trust can be found on qt's website at www.dcqualitytrust.org. Quality trust acts as a fiscal agent for project action!, a regional coalition of self-advocates and self-advocacy groups from the district of columbia and surrounding areas. Quality trust receives and distributes funds raised in compliance with the mission of quality trust and the legal requirements of qt's 501(c)(3) status.

PART X, LINE 2:

For the year ended september 30, 2024, quality trust has documented its consideration of fasb asc 740-10, income taxes, that provides guidance for reporting uncertainty in income taxes and has determined that no material uncertain tax positions qualify for either recognition or disclosure in the financial statements.

THIS YEAR STAFF TRIAGED 1220 SERIOUS REPORTABLE INCIDENTS, 123 RANDOMLY DRAWN PEOPLE FOR MONITORING ASSIGNMENTS, 400 INDIVIDUAL SURVEYS IN THE NATIONAL CORE INDICATORS PROJECT (NCI) AND SUPPORTED 15 NEW PEOPLE THROUGH ADVOCACY. WE MET PEOPLE LIVING IN HOST HOMES, SUPPORTED LIVING, ICFS, NATURAL HOMES AND LTAC FACILITIES.

OUR WORK IS ACCOMPLISHED BY (1) PROVIDING INDIVIDUAL AND GROUP ADVOCACY SUPPORT, (2) PROVIDING TRAINING TO PEOPLE WITH DISABILITIES, FAMILIES, AND PROFESSIONALS, (3) PROVIDING EDUCATION AND ADVOCACY FOR SPANISH-SPEAKING PERSONS, AND (4) WORKING IN COLLABORATION WITH SEVERAL GOVERNMENT AND COMMUNITY ORGANIZATIONS. WE UTILIZE VIRTUAL MEETING PLATFORMS TO CONDUCT SOME OF OUR WORK AS THEY FACILITATE ADDITIONAL MEETING PARTICIPATION WITHOUT THE NEED TO TRAVEL TO VENUES AND ALLOW PEOPLE TO OVERCOME CHALLENGES TO JOINING IN-PERSON. THIS YEAR, OUTREACH AND SPECIAL PROJECTS STAFF PARTICIPATED IN 10 FORMAL COLLABORATIONS, SUPPORTED 57 PEOPLE WITH DISABILITIES AND 157 PARENTS AND CAREGIVERS. WE FACILITATED 16 VIRTUAL TRAININGS AND WEBINARS, AND 270 PEOPLE WERE IMPACTED BY OUTREACH AND SPECIAL PROJECTS.

THEY ACCOMPLISH THIS BY PROVIDING (1) INDIVIDUAL AND SYSTEMIC LEGAL ADVOCACY TO PROMOTE SELF-DETERMINATION AND IMPROVE ACCESS TO HIGH-QUALITY SERVICES AND SUPPORTS, (2) PROVIDING EDUCATION, OUTREACH, AND TRAINING TO PEOPLE WITH DISABILITIES, FAMILIES, LEGAL AND DISABILITY PROFESSIONALS IN DIFFERENT DISABILITY POLICY AND PRACTICE AREAS, AND (3) DIRECTING SEVERAL SPECIFIC PROJECTS ON UTILIZING BEST PRACTICES TO SUPPORT PEOPLE WITH DISABILITIES.THIS YEAR STAFF CONDUCTED 15 TRAININGS AND PRESENTATIONS BY THE LEGAL TEAM IN 2024, OVER 350 PARTICIPANTS (FAMILY MEMBERS, PEOPLE WITH DISABILITIES, AND PROFESSIONALS), AND AT LEAST 40 STAKEHOLDER MEETINGS (INCLUDING MEETINGS WITH LOCAL AND NATIONAL ADVOCATES, MONTHLY MEETINGS WITH DDS, AND WORKING GROUP MEETINGS).
