Cessation of or significant change to any program service Part III line 3

Effective June 1, 2013, Health for Friends entered into an affiliation agreement with Variety Care, Inc., an Oklahoma not-for-profit corporation. The assets (except for cash of $7,843) and liabilities of Health for Friends were transferred to Variety Care, Inc. and the operations of Health for Friends became a part of the operations of Variety Care, Inc. The remaining cash was transferred to Variety Care, Inc. in 2014.
