“At every fiscal year end, St. John Health System, Inc. ("SJHS") distributes a copy of the current Conflict of Interest Policy and procedure bulletin, together with an explanation and questionnaire to the members of the board of directors, administrative officers and key employees of SJHS, its subsidiaries and affiliates, including St. John Auxiliary, Inc. The board members, administrative officers and key employees of SJHS, its subsidiaries and affiliates must complete the questionnaire and return it to the designated SJHS official within two weeks of receipt. Complete questionnaires are reviewed and summarized by the Vice President, Corporate Compliance and Integrity, or his/her designee. That individual then presents the questionnaire results to the heads of each hospital for further provision to the various boards' audit and compliance committees. The audit and compliance committees, as appropriate, submit a confidential report to their board chairman summarizing the questionnaire results. The board chairman, as appropriate, may review with the executive committee the responses to the questionnaire results.”

| IRS990/Desc | 0 | The St. John Auxiliary, Inc. has services at the Medical Center in Tulsa, Owasso, Broken Arrow, and Sapulpa. On average we have 218 adults who actively serve hours, and in this last fiscal year they donated 55,855 hours of their time. Volunteers serve in many vital areas including guest services/information desks, surgery waiting, emergency rooms, pastoral care, gift shops, flowers and dismissals, and on patient floors. In addition, the St. John Auxiliary, Inc. places emphasis in youth education. Students have the opportunity to learn about careers in the medical field through service, hands-on seminars, and guest speakers who share their expertise. 390 students from 37 area high schools and some home-schooled students served either during the school year program, the summer program, in internships, or Club Scrub. The students served 20,689 hours during this past fiscal year and served in lots of areas such as emergency room, transport, radiology, and on patient floors. Nearly all the money that we make is because of the efforts of the volunteers in the gift shops. Last year, we finished our pledge for the upgrades to the Da Vinci Surgical Robot and we have begun our pledge for Telemedicine equipment - in particular, Tele Stroke. We are proud to support the patients, families, physicians, and staff of St. John Health System. |

That individual then presents the questionnaire results to the heads of each hospital for further provision to the various boards' audit and compliance committees.
