Mission

Ownership and operation of hospitals, assisting living facilities and nursing homes in a charitable manner and the provision of health care related services in a charitable manner, and including for such purposes the making of distributions and contributions to organizations described in internal revenue code section 501(c)(3) and exempt from taxation under internal revenue code section 501(a).

To serve the healthcare needs of our community and to enhance the quality of life for every resident, family member, visitor, and team member we encounter. We are dedicated to providing compassionate, person-centered care that promotes dignity, respect, and well-being.

The form 990 is completed annually and copies are provided to the entire governing board as well as the president of the organization. At that time the president reviews the form 990 with the board members. Any necessary changes are then updated on the form. Once all necessary changes are made and the president is in agreement with the board members on the finished form 990, it is signed by the president, dated and submitted by the filing deadline. A copy of the approved form 990 is provided to all of the officers, directors and trustees before the return is filed.

Financial Statement Notes

PART X, LINE 2:

Fhs, its subsidiaries, and its affiliates have been recognized as tax-exemptpursuant to section 501(c)(3) of the internal revenue code. No provision for income taxes hasbeen provided for federal and state income tax purposes. Fhs, its subsidiaries, and its affiliates account for the effects of any uncertain tax positions basedon a "more likely than not" threshold applied to the recognition of that tax position being sustainedbased on the technical merits of the position under scrutiny by the applicable taxing authority. If atax position or positions are deemed to result in uncertainties of those positions, the unrecognizedtax benefit is estimated based on a "cumulative probability assessment" that aggregates theestimated tax liability for all uncertain tax positions. Interest and penalties assessed, if any, areaccrued as income tax expenses. Fhs, its subsidiaries, and its affiliates have identified their tax status as tax-exempt entities as atax position; however, fhs, its subsidiaries, and its affiliates have determined that such a taxposition does not result in an uncertainty requiring recognition. Fhs, its subsidiaries, and itsaffiliates believe they are no longer subject to income tax examinations for years prior todecember 31, 2021.
