Mission

The Health Education Council, Inc. (HEC) is a California nonprofit corporation serving to implement programs that impact the underlying drivers of health education, neighborhood safety, social connection, economic well-being, access to healthy resources that affect a persons ability to achieve health and well-being in the community of Sacramento. The Organization offers programs to improve health and wellbeing for all who live and work in the Sacramento area through innovation and partnerships and community driven programming. Working with youth and their families, the Organization aims to break cycles of poverty and improve quality of life for those served. For more than 30 years, addressing the social drivers of health in underserved communities has been the foundation of the Organizations work. IRS990/Desc0Program Area: Health Access Place-Based Health Planning and Data UseAs lead for Invest Health Roseville, HEC convened partners to improve health and quality of life in Downtown Rosevilles core neighborhoods.In 20242025, the advisory committee:Met monthly to align resources and attract investments.Supported the redesign and reopening of Weber Park in May 2025, based on resident input.Helped launch an Accountable Community for Health (ACH) in Placer County.Through the City Health Dashboard Data Challenge, HEC and partners examined neighborhood-level data on 40 health measures related to jobs, workforce development, and health. Findings were shared with 75+ local stakeholders and informed future investments.Accountable Communities for Health (ACH) West Sacramento and PlacerHEC served as backbone convener for two Accountable Communities for Health (ACH) initiatives.West Sacramento ACH focused on cardiovascular health, healthy food access, physical activity, social connection, and later, COVID-19 response and food insecurity. In 20242025, HEC:Partnered to conduct listening sessions on social connection and isolation.Used findings to integrate social connection into an expanded mobile farmers market, a Walk with Friends site at Westfield Elementary, and community meals using recovered food.Placer County ACH Network focused on improving well-being and supporting implementation of CalAIM (California Advancing and Innovating Medi-Cal).Led learning sessions on CalAIM for providers.Connected community-based organizations and health care partners.Convened a steering committee of 24 organizations.Organized a CalAIM convening in December 2024 for 65 participants, focused on Enhanced Care Management and Community Supports.The ACH also began planning for sustainability beyond 2026 and developed clear outreach materials on CalAIM benefits.Emergency Preparedness and Health OutreachHEC delivered disaster preparedness, response, recovery, and mitigation education to Spanish-speaking immigrants, older adults, and farmworkers in Sacramento, Yolo, Solano, and Placer Counties.Staff reached 400+ individuals per week through daily presentations at the Mexican Consulate in Sacramento and additional community events.HEC hosted listening sessions on how to build more prepared communities and gathered recommendations on training, workshops, and emergency alert tools.Overall, HECs emergency preparedness outreach reached 64,267 individuals.HEC also supported Healthy Air Quality work in partnership with the Placer County Air Pollution Control District (PCAPCD), reaching 4,000+ community members with information on air quality, health impacts, and local incentive programs.Food Access and Housing-Based Health SupportsTo address food insecurity, HEC partnered with Placer, Sacramento, and Yolo Food Banks to increase access to fresh, healthy foods:A bi-monthly food distribution at Mack Road Valley Hi Community Center served an average of 1,210 residents each month.A monthly distribution at four senior communities in Roseville served 120 residents monthly.A mobile farmers market in West Sacramento served 4,400 individuals at four low-income neighborhood sites.Across all efforts, HEC distributed approximately 200,000 pounds of produce and non-perishable food and shared recipes, budgeting tips, and local food resource lists.HEC also worked with the West Sacramento Housing Development Corporation to support health, social connection, and financial well-being in subsidized apartment communities:Provided education on nutrition, mental health, and financial literacy.Partnered with Yoga Moves Us to offer accessible physical activity.Hosted resident-led art classes.Distributed newsletters to all units, reaching 600+ residents.Coordinated monthly food deliveries with Yolo Food Bank to 145 households across six housing complexes.Health Navigation, Trusted Messengers, and Care ManagementAt the Ventanilla de Salud (VDS) at the Mexican Consulate, HEC staff served as trusted nav

(HEC) is a California nonprofit corporation serving to implement programs that impact the underlying drivers of health education, neighborhood safety, social connection, economic well-being, access to healthy resources that affect a persons ability to achieve health and well-being in the community of Sacramento. The Organization offers programs to improve health and wellbeing for all who live and work in the Sacramento area through innovation and partnerships and community driven programming. Working with youth and their families, the Organization aims to break cycles of poverty and improve quality of life for those served. For more than 30 years, addressing the social drivers of health in underserved communities has been the foundation of the Organizations work. IRS990/ProgSrvcAccomActy2Grp/Desc0Community Safety Community Safety & Violence PreventionSHOP Youth Program and Exposure ExperiencesThe Safety, Health, Opportunity, and Practice (SHOP) Youth Program supported youth ages 1220 facing academic, social, or behavioral challenges across Yolo and Sacramento Counties.SHOP used a healing-centered approach to help youth:Process adverse childhood experiences.Build emotional regulation and communication skills.Strengthen conflict resolution and social skills.Develop financial literacy and work readiness.Understand the role of social and environmental factors in trauma and justice.From July 2024 to June 2025, SHOP:Hosted 17 cohorts (six to eight weeks each).Served 215 youth.Delivered more than 1,436 hours of health education, crisis intervention, and mentorship.HEC also launched SHOP: Just Add Water, an exposure-based extension led in collaboration with local fishing expert and youth mentor Robert Nakatomi. The program provided fishing trips, nature excursions, and life skills education to youth ages 1217 through eight Power of Exposure trips, serving 60 youth.The Community Safety team began expanding SHOP to additional sites, including Marina Vista, Alder Grove, Venture Lab Roseville, and Valley High School Health Tech Academy.Collective Violence Prevention and Youth LeadershipAs backbone organization for the Sacramento Youth Violence Prevention Collective (SYVPC), HEC coordinated a multi-sector network of more than 100 partnerslaw enforcement, schools, hospitals, health departments, community-based and faith-based organizations, and universities.HEC:Organized 12 convenings per year, alternating between full collective meetings and Youth Commissioner meetings.Provided training and educational opportunities for partners and residents.Supported development and implementation of a shared violence prevention logic model.The Youth Commissioner Practicum offered young people a platform to inform strategies and apply public health theory to real community work. IRS990/ProgSrvcAccomActyOtherGrp/Desc0Social Connection Lay Mental Health Training and Community ProjectsThe Peers Helping Peers (PHP) initiative prepared youth and Spanish-speaking adults to reduce mental health stigma, build social connection, and pursue entry-level mental health roles.In 20242025, HEC led PHP cohorts for Spanish-speaking adults in three counties:Solano County: 2 trainings, 29 graduates, 6 community projects, and 5 participants engaged in outreach.Sacramento County: 1 training, 17 graduates, 8 community projects.Placer County (City of Lincoln): 1 training in July 2024, 14 graduates.Participant-led projects included youth soccer practices, crochet and piata classes, yoga, Harvesting Benefits for Your Happiness, embroidery circles, parent support programs, and anti-bullying campaigns.As part of Solano Countys Community Health Improvement Plan (CHIP), HEC used the PHP model to support Latino residents in Vacaville, Dixon, and Rio Vista through community mental health navigators, support groups, and presentations.Individual Mental Health Support and Early InterventionMente Sana, Vida Sana focused on Latino immigrant communities using culturally relevant screening tools and counseling.Between July 2024 and June 2025, the program:Conducted 4,933 health screenings.Completed 523 PHQ-9 depression screenings.Provided 5,819 individualized education and support sessions.Served 290 individuals with mental health services or referrals.Participants typically attended 57 one-hour sessions with Spanish-speaking counselors, and many reported feeling more hopeful and better able to cope with their circumstances.Through Prevention & Early Intervention (PEI) activities, HEC also provided mental health and social support to Afghan refugee communities, Latinx communities, and youth. In 20242025, HEC:Served 652 individuals with individualized services.Reached 20,500 people through outreach and group activities.Participated in 410 outreach events across Sacramento County.Social Connection, Physical Activity, and Health EducationWalk with Friends walking groups fostered both physical activity and social connection at:Robla Park (North Sacramento)Swanston Park (Arden-Arcade)Elkhorn Park (West Sacramento)Westfield Park (new site)Across sites, Walk with Friends averaged 130 participants per week. IRS990/ProgSrvcAccomActyOtherGrp/Desc1Economic Well-Being Financial Education and NavigationThrough the Ventanilla de Asesora Financiera y Proteccin al Patrimonio (VAF) at the Mexican Consulate in Sacramento, HEC provided financial education and resource navigation for low-income community members.In 20242025, HEC:Reached over 34,000 community members through in-person and online workshops, monthly fairs, and daily presentations.Provided individual navigation to 2,041 people.Reached 20,889 people through Facebook Live sessions.Reached 466 individuals through financial education workshops.Engaged 10,780 individuals at monthly resource fairs and ongoing presentations.Topics included banking, home ownership, saving and budgeting, fraud protection, unemployment support for migrant and seasonal farmworkers, employment and retirement resources, and financial tools available in MexicoHEC also provided community financial literacy workshops in English and Spanish at sites such as Midtown Church, Robla Community Park, Elkhorn Village Elementary, Swanston Community Center, Westfield Elementary, Capital College & Career Academy (CCCA), Logan Park Apartments, and the HEC office, reaching 200+ participants.Workforce Partnerships and EventsThe Workforce Development Advisory Board met monthly, bringing together employers, chambers of commerce, health providers, nonprofits, faith communities, financial institutions, and education partners. The Board focused on addressing employment as a social driver of health and helped plan workforce events.In September 2024, HEC hosted a Workforce Development Symposium & Job Fair at Midtown Church:The symposium drew 120+ attendees, including elected officials, business and nonprofit leaders, and government representatives, and explored links between workforce development and community health.The job fair featured 33 vendors and attracted 400+ job seekers.An on-site computer lab supported rsum development and job applications.Micro-Lending and Small Business SupportHEC launched a micro-lending pilot program to support small business owners, in partnership with:California Black Chamber of Commerce (CBCC) Inclusivity ProjectSacramento Hispanic Chamber of Commerce (SHCC) EmprendeBlack Small Business Association of California (BSBA) Business BoostHECs financial health programming at the Mexican ConsulateDuring outreach:Over 150 individuals received information on micro-loans and small business development.15 candidates were screened, 11 interviewed, and 8 selected for loans administered by Self-Help Federal Credit Union (SHFCU).Recipients included a street food vendor, custom clothing stylist, vehicle wraps and apparel business, low-voltage technologies business, Teff grain farm, Peruvian crafts and food products business, fitness program consultant, and preschool operator.Most participants maintained on-time payments, improved credit, and completed small business training programs. All were connected to the Sacramento Valley Small Business Development Center (SBDC), and most received one-on-one financial coaching. HEC also hosted a marketing workshop with 3fold Communications and coordinated technical assistance and referrals on bookkeeping, licensing, taxes, and business management.Career Readiness and Workforce PathwaysHEC launched a Career Readiness Cohort for high school juniors and seniors in Sacramento, Placer, and Yolo Counties.At Yolo High School in fall 2024:8 students participated in a 5-week program with two sessions per week.Topics included career exploration, job search skills, rsum building, dress for success, conflict resolution, interview preparation, mock interviews, panel discussions, and financial literacy.Post-survey results showed:90% felt more prepared for job interviews.80% better understood how to search for careers.90% knew what to include in a rsum.70% knew how to create a budget.80% felt more confident applying for entry-level jobs.90% knew how to dress for interviews.A second cohort was planned at Liberty Towers Youth C
