WE HELP PARENTS OF NEWBORNS UNDERSTAND THE IMPORTANCE OF DOCTOR'S VISITS, PROPER NUTRITION AND PREPARING THEIR HOME FOR THE BABY'S ARRIVAL. OUR FOCUS IS EDUCATING THE PARENT ON EVERY ASPECT OF PARENTING. BY TEACHING PARENTS ABOUT CHILD DEVELOPMENT, AGE APPROPRIATE ACTIVITIES AND APPROPRIATE DISCIPLINE TECHNIQUES, OUR STAFF MEMBERS HELP BREAK GENERATIONAL CYCLES OF CHILD ABUSE. 100% OF THE CHILDREN SERVED ONE YEAR AFTER COMPLETION OF SERVICES WERE STILL FREE OF ABUSE AND NEGLECT. 99% OF THE CHILDREN SERVED WERE FREE FROM ABUSE AND NEGLECT DURING SERVICE IN FISCAL YEAR 2016. HEALTHY FAMILIES SERVED 382 FAMILIES INCLUDING 864 CHILDREN AND 391 ADULTS IN 2016. HEALTHY FAMILIES IS NATIONALLY ACCREDITED BY HEALTHY FAMILIES AMERICA WITH COST SAVINGS UP TO $102,960 FOR EVERY CHILD REMAINING WITH THEIR FAMILY AND OUT OF THE CHILD WELFARE SYSTEM.

THE GOAL OF TOPWA IS TO LOWER THE NUMBER OF BABIES BORN WITH PRENATAL DRUG EXPOSURE AND HIV INFECTION. THE PROGRAM OFFERS SUPPORTIVE, EDUCATIONAL AND CLIENT-CENTERED OUTREACH SERVICES TO LOWER THE RISK FOR HIV INFECTION OR SUBSTANCE ABUSE. IN 2016, 215 HIGH-RISK PREGNANT WOMEN WERE SERVED. 6 BABIES BORN TO HIV+ WOMEN WERE BORN HEALTHY AND TESTED NEGATIVE FOR THE VIRUS AFTER SIX MONTHS FOLLOW-UP. SINCE 1999, 100% OF THE CHILDREN BORN TO HIV+ WOMEN HAVE TESTED NEGATIVE. COST SAVINGS: $21,768 PER YEAR FOR EVERY CHILD BORN FREE FROM HIV.

PROGRAM STAFF COUNSEL ON A WIDE VARIETY OF ISSUES AND CONCERNS, NOT THE LEAST OF WHICH INCLUDE OBTAINING ECONOMIC BENEFITS, EDUCATIONAL SERVICES, LEGAL SERVICES, FINANCIAL GUIDANCE AND HEALTH INSURANCE. ACCESS TO SUPPORT GROUPS FOR RELATIVE CAREGIVERS AND THERAPEUTIC SERVICES ARE OFFERED. IN 2016, THE KIN SUPPORT PROJECT SERVED 50 FAMILIES INCLUDING 111 CHILDREN AND 62 ADULTS. 106 CHILDREN REMAINED WITH RELATIVE CAREGIVERS AND OUT OF THE FOSTER CARE SYSTEM, A 95% SUCCESS RATE. COST SAVINGS: $102,960 FOR EVERY CHILD REMAINING WITH FAMILIES AND OUT OF THE CHILD WELFARE SYSTEM.

THE PROGRAM IS GEARED TOWARDS WORKING WITH VERY VULNERABLE YOUNG CHILDREN AND FAMILIES TO PROVIDE INTENSIVE, HOME-BASED SERVICES. CHILD FIRST WORKS TO DECREASE THE INCIDENCE OF SERIOUS EMOTIONAL DISTURBANCE, DEVELOPMENTAL AND LEARNING PROBLEMS, AND ABUSE AND NEGLECT AMONG HIGH-RISK YOUNG CHILDREN AND FAMILIES. CHILD FIRST PROVIDES SERVICES TO FAMILIES WITH CHILDREN BIRTH TO AGE 6 IN WHICH THE CHILD HAS EMOTIONAL, BEHAVIORAL, OR DEVELOPMENTAL CONCERNS OR THE FAMILY FACES MULTIPLE BARRIERS. A CLINICIAN AND CARE COORDINATOR PROVIDE SERVICES THAT INCLUDE COMPREHENSIVE ASSESSMENT OF CHILD AND FAMILY NEEDS, OBSERVATION AND CONSULTATION IN EARLY CARE AND EDUCATION SETTINGS, PARENT-CHILD MENTAL HEALTH INTERVENTION, DEVELOPMENT OF A FAMILY AND A CHILD PLAN OF CARE, AND CARE COORDINATION/CASE MANAGEMENT. IN 2016, CHILD FIRST SERVED 50 FAMILIES, 9 OF WHICH RECEIVED SERVICES PRENATALLY.CHILDREN'S BEHAVIORAL HEALTH COLLABORATIVE WAS IMPLEMENTED IN DECEMBER 2014. FIVE AGENCIES, INCLUDING FAMILIES FIRST, PROVIDE INDIVIDUALIZED, COMPREHENSIVE, FAMILY-DRIVEN MENTAL HEALTH SERVICES FOR AT-RISK AND HIGH-RISK CHILDREN AND THEIR FAMILIES ACROSS PALM BEACH COUNTY. THIS NATIONAL ACCREDITED PROGRAM THROUGH THE COUNCIL ON ACCREDITATION FOR CHILDREN AND FAMILIES HAS EXPANDED TO INCREASE IN THERAPISTS FROM ONE TO THREE AS WELL AS ADDING A CLINICAL DIRECTOR TO OVERSEE THE BEHAVIORAL HEALTH SERVICES. SERVICES HAVE ALSO EXPANDED TO COVER THE ENTIRE PALM BEACH COUNTY INCLUDING THE WESTERN COMMUNITIES IN THE GLADES. IN ADDITION, THE AGENCY HAS BECOME A MEDICAID PROVIDER. IN 2016, FAMILIES FIRST SERVED 33 FAMILIES THAT INCLUDED 86 CHILDREN AND 62 ADULTS. COST SAVINGS: $2,776.85 PER CHILD, BASED ON REDUCED DAYS OF HOSPITALIZATION.PROJECT CONNECT PROVIDES COMMUNITY-BASED CARE COORDINATION SERVICES FOR FAMILIES WHO HAVE MULTIPLE ISSUES AND NEEDS THAT IMPACT THEIR ABILITY TO MAINTAIN STABILITY AND SELF-SUFFICIENCY. THE PROJECT CONNECT COLLABORATIVE IS A UNIQUE PROJECT THAT DEVELOPED OUT OF THE GAP IN SERVICES FOR HOMELESS PREVENTION. ONE IN SEVEN FAMILIES WITH CHILDREN UNDER THE AGE OF FIVE LIVE IN POVERTY. PROJECT CONNECT TARGETS WORKING FAMILIES, WHO AS THE RESULT OF SITUATIONAL CRISIS, ILLNESS, JOB LOSS, OR OTHER LIFE CHANGING EVENTS, FIND THEMSELVES HOMELESS OR AT RISK OF HOMELESSNESS. PROGRAM GOALS ARE FOCUSED ON FAMILIES ACHIEVING A LEVEL OF FINANCIAL MANAGEMENT NECESSARY TO BE SELF-SUFFICIENT. THE PROGRAM IS SUCCESSFUL BY STABILIZING FAMILIES AND GIVING THEM THE TOOLS NEEDED TO LIVE WITHIN THEIR BUDGET. IN 2016, PROJECT CONNECT SERVED 25 FAMILIES WHICH INCLUDED 62 CHILDREN AND 38 ADULTS. SAVINGS: UP TO $8,796 PER YEAR FOR EVERY FAMILY PREVENTED FROM ENTERING AN EMERGENCY SHELTER.BRIDGES TO SUCCESS IS A COMMUNITY-BASED COLLABORATIVE PROJECT BETWEEN ADOPT-A-FAMILY AND FAMILIES FIRST. THE PROJECT FOCUSES ON PROVIDING TEN UNITS OF PERMANENT HOUSING WITH SUPPORTIVE SERVICES TO FAMILIES IN BELLE GLADE, PAHOKEE AND ROYAL PALM BEACH, ALL IN PALM BEACH COUNTY. THE PROGRAM COORDINATOR HELPS EACH FAMILY SET THEIR OWN GOALS TO BECOMING SELF-SUFFICIENT. THIS PROGRAM BECAME NATIONALLY ACCREDITED THROUGH THE COUNCIL ON ACCREDITATION FOR CHILDREN AND FAMILIES IN MARCH 2012. IN 2016, 10 FAMILIES, INCLUDING 13 ADULTS AND 15 CHILDREN, RECEIVED HOUSING AND SUPPORTIVE SERVICES. COST SAVINGS: IT COSTS APPROXIMATELY $31,000 A YEAR FOR HOMELESS SERVICES; PERMANENT SUPPORTIVE HOUSING COSTS $10,000 A YEAR. COST SAVINGS $21,000 FOR EACH FAMILY.
