In addition, the form 990 is reviewed in detail by the associate director and by the treasurer of the board. Form 990, page 6, part vi, section b, line 12c: each member of the board of directors is required at the beginning of each fiscal year to sign a conflict of interest disclosure statement attesting to any positions held, or interests in, any potential contracting parties, whether held by the director, or to the extent of the director's knowledge, by members of his or her immediate family (parents, children, spouse, or siblings).
