Form 990, Part VI, Section A, Line 2

Jennifer s. Maddox, president & ceo of the foundation, is a minority shareholder in a board member's (tommie goggans) company.

Form 990, Part VI, Section B, Line 11B

Board members are provided copies prior to the board meeting where the ceo, cfo and accounting firm present a copy of the form 990 for detailed review by the organization's board prior to filing.

Form 990, Part VI, Section B, Line 12C

Financial Statement Notes

PART V, LINE 4:

The foundation's intended use of the endowment funds is to provide support in perpetuity to the beneficiary organization or charitable purpose as provided for in the document creating the endowment.

PART X, LINE 2:

The foundation applies the provisions of the accounting standards codification relating to uncertainty in income taxes. The guidance required entities to assess their uncertain tax positions for the likelihood that they would be overturned upon irs examination. In accordance with this guidance, the foundation had determined that it does not have any positions at september 30, 2020 and 2019, that it would be unable to substantiate.

Part V, Line 1E

Opening balances for the year ended september 30,2018 were adjusted to reflect a change in presentation of funds on the prior year audit. All accumulated changes effecting the endowment were recorded on line 1e in the prior year activity of schedule d, part v. Ending endowment totals include without donor restrictions: board endowed and with donor restrictions: by spending policy & in perpetuity.
