Big bend hospice (bbh) is the sole member of big bend hospice foundation (foundation). Bbh shall appoint two trustees to the foundation board of trustees.

Bbh shall appoint two trustees to the foundation board of trustees based on the terms of its operating agreement and existing by-laws.

A final draft of the full form 990, including all applicable schedules, is presented to the executive committee by our tax advisors. A copy is then provided to each remaining board member prior to filing with the irs.

At the time of appointment and at least annually thereafter (or at the time of occurrence), board members and executive staff complete an annual disclosure statement to disclose any business or other employment relationships which represent potential conflicts of interest with the organization. If there is a potential conflict of interest, the executive director, board chairperson, and audit committee review the statement and determine if there is an actual conflict of interest. Board members are required to abstain from voting on matters which present a conflict of interest as described above and shall excuse themselves from the meeting during discussion and the voting process.

The process for determining the compensation of the organization's executive director includes a review by the executive committee of the board of trustees. Compensation surveys are used to determine the amount of compensation. The last review took place in fy 2014 and was documented in the minutes of the executive session.

Schedule D, Part V, Line 4, Intended uses of endowment funds

Big bend hospice foundation, inc. Possesses endowment funds in the form of long-term investments whose earnings are intended to support the mission of big bend hospice. Big bend hospice foundation has adopted investment and spending policies that attempt to provide a perpetual source of support to programs supported by the endowment while seeking to preserve the purchasing power of the endowment assets.

Schedule D, Part X, Line 2, FIN 48 (ASC 740) footnote

The organization is exempt from income taxes on income from related activities under section 501(c)(3) of the u.s. Internal revenue code and corresponding state law. Accordingly, no provision has been made for federal or state income taxes. Additionally, the organization has been determined not to be a private foundation under section 509(a) of the internal revenue code. U.s. Gaap requires that a tax position is recognized as a benefit only if it is "more likely than not" that the tax position would be sustained in a tax examination, with a tax examination being presumed to occur. The amount recognized is the largest amount of tax benefit that is greater than 50% likely of being realized on examination. For tax positions not meeting the "more likely than not" test, no tax benefit is recorded. Due to its tax-exempt status, the organization is not subject to u.s. Federal income tax or state income tax. The organization's form 990 has not been subject to examination by the internal revenue service or the state for the last three years. The organization does not expect the total amount of unrecognized tax benefits to significantly change in the next 12 months. The organization recognizes interest and/or penalties related to income tax matters in income tax expense. The organization did not have any amounts accrued for interest and penalties at september 30, 2014 and 2013.

PROFESSIONAL COUNSELORS LISTEN, TEACH, AND PROVIDE STRATEGIES FOR THESE CHILDREN TO SUCCESSFULLY NAVIGATE THE GRIEVING PROCESS. MUSIC THERAPY OFFERS PRESCRIBED USE OF MUSIC BY TRAINED PROFESSIONALS TO ATTAIN SPECIFIC THERAPEUTIC GOALS & ENHANCE PATIENT CARE.
