“All officers, directors, trustees, key employees and highest paid employees (interested persons) of the organization have a duty to avoid conflicts of interest, both real and perceived, which may negatively impact the organization or those it serves. The organization's interested persons are to be guided by the organization's mission, vision and values and to serve patients, families and the general public without need for any personal favor or gain. The organization's ethics and compliance plan emphasizes the duty interested persons have to disclose any actual or potential conflicts of interest that may benefit their private interests or result in a possible excess benefit transaction. Conflicts are disclosed annually on a conflict of interest questionnaire that is distributed to the officers, directors, key employees, and highest compensated employees. In the event of any actual or potential conflicts of interest, interested persons must disclose the existence of their financial interest and disclose all material facts to the board chair, ceo or other designated persons. If it is determined an actual conflict of interest exists between the organization and an interested person, the party with a conflict of interest must abstain from any discussion or voting on the transaction or arrangement involving the conflict of interest. At each board meeting, board members are reminded that they have signed a conflict of interest disclosure and are asked to review the agenda and, for the record, disclose any items with which they may have a conflict of interest.”

“(continued from part iii) the suncoast hospice institute's online education has helped to shape the end-of-life and palliative care provided to people around the globe. The learn center provides its education to employees and volunteers who can learn at their own pace. The online learning programs include training in hipaa, infection control, restraints, ethics and compliance, and all of the material covered is designated to result in better care for the patients. The online learning course list is constantly growing and offers hospice and palliative care courses, projected courses and computer software and technology online courses. All of the material was developed by practice-based content experts and e-learning instructional designers to be interactive, engaging, promote retention and address varied learning styles. Unlike other learning management systems, these courses accommodate instructor-led training. The suncoast institute's resource library features a wide variety of materials. Some of the items available are job aids, educational sheets for patients and families, as well as other resources for clinical and nonclinical staff and volunteers. In addition to its array of learning materials and resources, suncoast hospice institute provides consulting services and other products to the hospice community.”

“The hospice institute of the florida suncoast, inc. Is included in consolidated financial statements. Below are the fin 48 paragraphs from the financial statements that specifically address the hospice institute of the florida suncoast: empath's affiliates (except solutions) are exempt from income taxes on income from related activities under section 501(c)(3) of the u.s. Internal revenue code and corresponding state tax law. Accordingly, no provision has been made for federal and state income taxes for the entities except for solutions. Solutions is a c corporation, and is subject to u.s. Federal income tax and state income tax in the state of florida. Due to its tax-exempt status, empath (except solutions) is not subject to u.s. Federal income tax or state income tax. Empath and its not-for-profit affiliates forms 990 have not been subject to examination by the internal revenue service or the state of florida for the last three years. Empath does not expect the total of unrecognized tax benefits to significantly change in the next 12 months. Empath recognizes interest and/or penalties related to income tax matters in income tax expense. Empath did not have any amounts accrued or recognized for interest and penalties at september 30, 2014 and 2013.”
