Included in this disclosure are organizations, partnerships, corporations, businesses or other legal entities, whether for profit or not-for-profit, in which the undersigned is a director, trustee, officer, partner, employee and/or owns or controls more than 5% of the voting securities. These forms are completed, signed and turned into the ceo who reviews and files the disclosures. Any potential conflict is brought to the attention of the board of trustees and disclosed. Potential conflicts are also brought to the attention of key staff members and legal counsel to determine the appropriate actions that need to take place to remain in compliance with all rules and regulations.

Form 990, Part VI, Section B, Line 15

The board of trustees appoints a compensation committee that reviews compensation of officers and key employees. The compensation of the ceo is included in a written employment contract and is approved by the board.
