Part VI, Section B, Line 11B

Once the form 990 has been prepared by an independent cpa, the finance committee will receive a copy of the return. It will be reviewed for accuracy and any recommendations for changes will be made. Once approved by the finance committee, the 990 will be sent to each of the board members for approval with a memo attached acknowledging finance committee's approval. Once board member approval is attained, the 990 will be finalized and processed for filing.

Future Foundation implements the following activities as part of our Second Family Model to ensure high quality access to education. We work with our partners to provide students with support in four key areas: academic support, building income, health, and family. 1) Academic: Future Foundation improves math and language arts outcomes through academic instruction, homework help, tutoring support, and STEAM enrichment activities. 2) Income: Future Foundation increases post-secondary readiness through college and career readiness, career-related enrichment activities, and financial literacy instruction. 3) Health: Future Foundation improves students' ability to express feelings, improve self-competence, decrease risky behavior, and increase physical activity through social emotional wellness instruction, sexual health education, and nutrition education. 4) Family: Future Foundation improves parent/child communication, increases knowledge of risky behaviors, and increases parent participation and engagement through parent workshops, town halls, and care box deliveries. 67% of Future Foundation students exhibit improved academic performance (measured by report card data) and 100% of our students are on track to graduate from high school on time. 57% of our parents attend at least one family engagement session focused on academic outcomes, which represents a 14% increase over the prior. 78% of Future Foundation youth show improvement in their social-emotional skills like emotional management, teamwork, empathy, responsibility, and problem solving (measured by pre- and post-surveys). Part V, Line 4

THE INTENDED USE OF THE ORGANIZATION'S ENDOWMENT FUNDS IS SPECIFIC PROGRAM INITIATIVES. THE FUNDS ARE COMPRISED OF PURPOSE OR TIMING RESTRICTIONS. PART X, LINE 1 In April 2020, the Organization obtained a Small Business Administration loan under the Paycheck Protection Program ("PPP") in the amount of $231,787. The PPP loan bears interest at 1.00% and may require repayment under certain circumstances. Under the terms of Coronavirus Aid, Relief, and Economic Securities Act (the "CARES Act") and the Paycheck Protection Program Flexibility Act ("PPPFA"), the Organization may apply with the lending institution for PPP loan proceeds used within a specified time period to be forgiven, provided the proceeds are used to cover certain payroll and other expenses as defined by the CARES Act and the PPPFA. Initial repayments of the loan amount are deferred until the date the SBA remits the loan forgiveness funds to the lending institution, or until 10 months after the end of the forgiveness coverage period if the Organization does not apply for forgiveness. The loan currently matures in April 2022. The Organization has recorded the loan as a conditional grant and has a refundable advance balance of $41,125 at December 31, 2020. Subsequent to year end, the Organization applied for partial forgiveness of the loan. The remaining refundable advance will be repaid.

Part X, Line 2

The Organization is exempt from federal income taxes under the provisions of Section 501(c)(3) of the Internal Revenue Code. Accordingly, no provision or liability for federal and state income taxes has been recorded in the accompanying financial statements. The Organization annually evaluates all federal and state income tax positions. This process includes an analysis of whether these income tax positions the Organization takes meet the definition of an uncertain tax position under the Income Taxes Topic of the Financial Accounting Standards Codification. In the normal course of business, the Organization is subject to examination by the federal and state taxing authorities. In general, the Organization is no longer subject to tax examinations for the years ending before December 31, 2017.
