To help physicians and their clinics with the challenge of monitoring the multiple medications of their medicare patients, novant health medical group physician practices operate safemed. Pharmacists help physicians identify and assess medication issues for their patients, as well as work directly with patients to answer medication questions. Safemed patients achieve higher rates of medication compliance, fewer drug misadventures, improved health outcomes and fewer readmissions to the hospital. Novant health medical group physician clinics are also leading new developments in how health services will be delivered in the future. In 2013, more than 100 clinics achieved patient-centered medical home national committee for quality assurance recognition. In this model of care, a physician-led team of nurses, physicians' assistants and others collaborate to ensure that every patient receives appropriate preventive and medical care, education and support. Novant health medical group physicians are also national leaders in achieving national committee for quality assurance recognition for demonstrating high-quality, disease specific care to patients with diabetes, heart conditions and stroke.
