Mission

Mountain states foundation ("msf") supports mountain states health alliance in healthcare and bringing loving care to healthcare. Msf's vision is to generate public support and awareness, encourage financial support, build endowments, and enlist the philanthropic efforts of other organizations. Through the efforts of friends, volunteers, and team members, msf provides support to a number of causes that are important to the people of northeast tennessee. Helping bring health to life, mountain states foundation advances quality healthcare in the northeast tennessee region by linking friends with mountain states health alliance and creating a difference through giving.

To support mountain states health alliance ("msha") in bringing loving care to healthcare.

The chairman of the board, msha president and chief executive officer and the msha corporate ethics and compliance officer shall become familiar with the statements of the board members in order to guide their conduct should a conflict arise. At such time as any matter comes before the board that may potentially result in a conflict of interest, the affected board member shall make known the potential conflict, whether or not disclosed in a written statement. The affected board member may make a presentation to the board or a committee meeting, but after such presentation, he or she must leave the meeting during the discussion of and the vote on the transaction or arrangement considered to be a conflict of interest.

PART V, LINE 4:

Endowment funds of $100,000 are used for nursing scholarships and nursing education - the income from these funds is recorded in a temporary restricted fund and distributed to assist nursing students. Endowment funds of $17,500 are held for the children's hospital, and the remaining $10,000 is held for pulmonary care.

PART X, LINE 2:

Mountain states foundation, inc. (the foundation) is classified as an organization exempt from federal income taxes under internal revenue code (irc) section 501(c)(3). Accordingly, no provision for income taxes has been included in the accompanying financial statements. In addition, the foundation has been determined by the internal revenue service not to be a "private foundation" within the meaning of section 509(a) of the irc. The foundation has no uncertain tax positions at june 30, 2016 and 2015. As such, no interest or penalties are recognized in the statement of activities related to uncertain tax positions for the years then ended. At june 30, 2016, informational tax returns for fiscal years 2013 through 2015 are eligible for examination by the internal revenue service.

FRANKLIN WOODS COMMUNITY HOSPITAL - THE FOUNDATION PROVIDED FUNDS FOR EQUIPMENT FOR A TEAM MEMBER WORKOUT ROOM. WOODRIDGE HOSPITAL - THE FOUNDATION PROVIDED FUNDS FOR NEW FURNITURE AND REMODELING OF THE ADOLESCENT "WILLOW UNIT."THE WASHINGTON COUNTY FOUNDATION ALSO PARTICIPATED IN A MATCHING GRANT FOR TELEMEDICINE.THE NISWONGER CHILDREN'S HOSPITALNISWONGER CHILDREN'S HOSPITAL SERVES A POPULATION OF MORE THAN 200,000 CHILDREN IN A 29-COUNTY REGION. THE HOSPITAL WAS DESIGNED TO PROVIDE FOR THE UNIQUE NEEDS OF CHILDREN AND COMBINES COMPASSIONATE, PERSONALIZED CARE WITH STATE-OF-THE-ART TECHNOLOGY. THE FOUNDATION PROVIDES FUNDING TO THE NISWONGER CHILDREN'S HOSPITAL EACH YEAR FOR VARIOUS NEEDS SUCH AS GIRAFFE ISOLLETTES, EEG EQUIPMENT, AND PEDIATRIC AMBULANCES, TO NAME A FEW.THE FOUNDATION CONTINUES TO PROVIDE ADDITIONAL PIECES OF EQUIPMENT AND OTHER HEALTH-RELATED NEEDS IN ITS ONGOING EFFORT TO PROVIDE PATIENT-CENTERED CARE TO THE PEDIATRIC PATIENTS.OTHER PROGRAMS SUPPORTED BY THE FOUNDATION:GROWING HEALTHYGROWING HEALTHY SERVES THE SIX SCHOOL SYSTEMS OF UPPER EAST TENNESSEE. THE PROGRAM IS COMMITTED TO PROMOTING HEALTHY LIFESTYLE CHOICES THROUGH ITS EFFORTS TO HELP SCHOOLS AND COMMUNITIES EFFECTIVELY LINK HEALTH WITH EDUCATIONAL ACHIEVEMENT AND IMPROVE STUDENTS' OVERALL WELL-BEING. GROWING HEALTHY FOCUSED ITS EFFORTS ON HELPING EDUCATORS DEVELOP QUALITY PLANNING/INSTRUCTION LESSONS THAT INTEGRATED HEALTH CONCEPTS WITHIN ALL CURRICULUM AREAS IN ORDER TO IMPROVE STUDENT HEALTH AND ACHIEVEMENT.TRAININGS WERE CONDUCTED FOR SECOND AND THIRD GRADE TEACHERS WHO ATTENDED SEVERAL THREE-DAY IN-SERVICE TRAININGS WHERE THEY RECEIVED INSTRUCTION ON HOW TO INCORPORATE A RESOURCE-RICH HEALTH CURRICULUM THAT USES A VARIETY OF PERIPHERAL MATERIALS TO ENGAGE AND CAPTIVATE THEIR STUDENTS IN THEIR CLASSROOMS. TEACHERS PARTICIPATING RECEIVED TRAINING ON HOW TO ENHANCE AND INCORPORATE THE HEALTH CURRICULUM WITH THE NEW STATE COMMON CORE STANDARDS AND THE INCREASING DEMANDS ON STATE TESTING SCORES.IN 2016, MOUNTAIN STATES FOUNDATION'S SPARK CURRICULUM EXPANDED TRAININGS FOR K-6 PHYSICAL EDUCATION TEACHERS IN THE SULLIVAN COUNTY SCHOOLS AND KINGSPORT CITY SCHOOLS. BY COLLABORATING WITH THE COORDINATORS OF SCHOOL HEALTH IN EACH NEW SCHOOL SYSTEM AND THE PREVIOUS SIX SCHOOL SYSTEMS, A TRAINING WAS SCHEDULED TO TRAIN NEW TEACHERS AS WELL AS REFRESH PREVIOUSLY TRAINED TEACHERS AND NEW HIRES SINCE THE ORIGINAL TRAINING IN 2008.
