For over 75 years, visiting nurse has served as georgia's first and largest nonprofit provider of health and hospice care in the home, and hospice atlanta being the first hospice in the state of georgia. From post-acute care, rehabilitation services, long-term care for chronic conditions, aging services, palliative and hospice care, visiting nurse provides the full continuum of care and aging services to over 13,000 patients each year, throughout 57 georgia counties in georgia, serving over 6,000 patients and their families each day.

Mission

Visiting nurse health system hospice atlanta offers a full continuum of home healthcare to the greater atlanta community. This includes post-acute care, rehabilitation services, long-term care for chronic conditions, and palliative and hospice care to nearly 12,000 patients annually to 31 counties in georgia. Our family of services includes home healthcare, community care, private home care, hospice and palliative care. For more than 72 years, the name visiting nurse health system has been synonymous with quality home care. Visiting nurse is the largest nonprofit home healthcare and hospice provider in georgia, serving nearly 6,000 patients and their families each day.

The mission of visiting nurse health system is to "improve the lives of those we serve."

Governance Explanations

Form 990, Part VI, Section A, Line 1A

Executive committee, which shall consist of two or more directors, shall have and exercise the authority of the board, the management of the corporation, but the designation of such committee and the delegation thereto of authority shall not operate to relieve the board or any individual director of any responsibilty imposed upon it or him by law. The executive committee shall keep regular minutes of its meetings and report the same to the board of directors when required.

The ceo and cfo approved the filing of the tax return. The form 990 was provided to the board of directors for their review prior to it being filed. The board was given the opportunity to ask questions of the internal preparers based upon this review prior to filing.

The organization requires all current board members or interested persons certify they have received, read, and agreed to comply with the conflict of interest policy and have disclosed any potential conflicts of interest annually.

Form 990, Part VI, Section B, Line 15

The compensation of the organization's officers and ceo is reviewed and approved by the organization's independent board of directors. Annually, the organization engages an outside consultant to compare compensation and benefits paid to the organization's officers and executive management to compensation and benefits that are paid to employees providing similar services by similar enterprises under similar circumstances. The consultant's report is reviewed by the consultant with the board. Staff compensation is monitored regularly against market rates for similar clinical and administrative positions with similar scope of responsibilities and adjusted when deemed necessary by executive management to ensure compensation is appropriate to the competitive marketplace. Annually, performance reviews are completed and merit increases are provided to staff based on individual performance and the financial ability of the organization as approved by the ceo and vp finance. Additionally, when deemed appropriate by executive management, certain clinical staff are provided temporary pay adjustments to appropriately compensate them for their work, based on increased scope of responsibilities or other relevant changes in circumstances.

Visiting nurse is exempt from federal income tax under section 501(c)(3) of the internal revenue code and, therefore, related income is not subject to federal or state income taxes. Management does not believe that visiting nurse or visiting nurse | hospice atlanta foundation, inc. Hold any uncertain tax positions that would require financial statement recognition or disclosure. Accordingly, no provision for income taxes has been made in the accompanying consolidated financial statements.

THIS CARE IS PROVIDED BY A TEAM OF APPROXIMATELY 135 PROFESSIONALS, SKILLED MEDICAL PERSONNEL INCLUDING NURSES, PHYSICAL THERAPISTS, OCCUPATIONAL THERAPISTS, SPEECH PATHOLOGISTS, HOME HEALTH AIDES AND SOCIAL WORKERS. THE PRIMARY GOALS OF BRINGING HEALTHCARE INTO THE HOME ARE TO HELP PATIENTS AVOID HOSPITAL READMISSION, IMPROVE THEIR CLINICAL OUTCOMES, AND IMPROVE THEIR OVERALL HEALTH FROM THE COMFORT OF THE PLACE THEY CALL HOME. IRS990/ProgSrvcAccomActy2Grp/Desc0VISITING NURSE HEALTH SYSTEM, UNDER THE HOSPICE ATLANTA BRAND, PROVIDES HOSPICE AND PALLIATIVE CARE IN THE HOMES OF PATIENTS WITH LIFE-LIMITING ILLNESSES WHO HAVE ELECTED TO STOP AGGRESSIVE, CURATIVE TREATMENTS. THE PRIMARY GOALS OF HOSPICE ATLANTA'S CARE IS TO PROVIDE QUALITY CARE, COMFORT, AND DIGNITY FOR PATIENTS DURING THEIR FINAL STAGE OF LIFE. IN ADDITION, WE PROVIDE PATIENTS' FAMILIES WITH THE EDUCATION, RESOURCES AND SERVICES NEEDED, ALONG WITH EMOTIONAL SUPPORT DURING THE DYING PROCESS, AND BEREAVEMENT SUPPORT AFTER THE DEATH OF THEIR LOVED ONE. OUR TEAM OF APPROXIMATELY 70 QUALIFIED PROFESSIONALS AND 80 SPECIALLY TRAINED VOLUNTEERS PROVIDE END-OF-LIFE CARE TO OVER 640 PATIENTS ANNUALLY. HOSPICE ATLANTA DELIVERS THIS COMPASSIONATE CARE IN THE COMFORT OF THE PATIENTS' HOME, THE PLACE THEY CHOOSE TO RECEIVE CARE. HOSPICE ATLANTA OFFERS A VARIETY OF EMOTIONAL AND SPIRITUAL BEREAVEMENT SUPPORT SERVICES, PROVIDED BY TRAINED BEREAVEMENT COORDINATORS AND BEREAVEMENT PROFESSIONALS, TO INCLUDE IN-PERSON COUNSELING, VIRTUAL SUPPORT VISITS, REGULARLY SCHEDULED GRIEF SUPPORT GROUPS, EDUCATIONAL RESOURCES, AND THE ANNUAL BEREAVEMENT EVENTS - A WORKSHOP AND FAMILY CAMP, CAMP S.T.A.R.S. BEREAVEMENT SUPPORT IS AVAILABLE FOR ONE YEAR AFTER THE DEATH OF A LOVED ONE, AND TWO YEARS FOR THOSE MOURNING THE LOSS OF A CHILD.

ALONG WITH THE COMMUNITY CARE TEAM, THE COMMUNITY CARE DIVISION PARTNERS WITH A BROAD RANGE OF COMMUNITY ORGANIZATIONS TO PROVIDE A COMPREHENSIVE RANGE OF SERVICES TO NURSING HOME-ELIGIBLE, LOW-INCOME SENIORS AND DISABLED INDIVIDUALS WHO HAVE ONGOING MEDICAL CONDITIONS REQUIRING LONG-TERM SUPPORT. THESE COMMUNITY CARE PROGRAMS INCLUDE THE SERVICE OPTIONS USING RESOURCES IN A COMMUNITY ENVIRONMENT PROGRAM (SOURCE), THE COMMUNITY CARE SERVICES PROGRAM (CCSP) AND THE HOME AND COMMUNITY BASED SERVICES PROGRAM (HCBS). A TEAM OF APPROXIMATELY 130 SOCIAL WORKERS AND NURSES WORK CAREFULLY TO DETERMINE EACH PATIENT'S INDIVIDUAL LONG-TERM CARE NEEDS, AND COORDINATE ACCESS TO CARE THROUGH A VARIETY OF LOCAL MEDICAL AND SOCIAL SERVICE RESOURCES FOR MORE THAN 6,000 SENIORS AND THEIR FAMILY MEMBERS EACH YEAR.
