IRS990/Desc0During the school year CISMC case managed approximately 220 students Grades 4-12. Case managed students have an individual plan containing measurable goals and recieve services twice a week for at least 1 hour per week. In addition to these students, CISMC provided tutoring services for another 100 students at Madison High School. Students at high at risk of failure are case managed students enabling them to graduate from high school, approximately 18% of a graduating class. The measurable objectives set for all case managed students and the end of year results are listed below:*80% of Madison Team Success Participants will have no new juvenile court complaints during program participation. Actual Percentage 98%*80% of Madison Team Success Participants will have no new adjudications during program participation. Actual Percentage 98%*75% of Madison Team Success Participants will successfully or satisfactorily complete services as measured by performance against an individual service plan. Actual Percentage 89%*80% Madison Team Success Participants completing the program will demonstrate a reduction in problem behaviors for which they were referred by termination. Actual Percentage 88%*80% of Madison Team Success Participants completing the program will demonstrate improvement in targeted skills as specified in the individual service plan by termination. Actual Percentage 86%During this fiscal year CISMC trained, supported and supervised 42 community volunteers who worked over 2600 hours directly with children.The Great Leaps Math program, a program designed to insure that elementary and middle school students have learned their basic math skills, was provided to over 100 4th-6th grade students.The Jump Start Program, a one day transition program provided prior to the start of the school year to in-coming 6th grade students making the transition from elementary to middle school students.
