Activity 2

As a Critical Access hospital we provided health care services to our community which included the following days of care: Skilled/ICF 12,434; Swing 1,588; and Acute Care 1,182. Additionally, we provided 28,278 outpatient diagnostic visits; 8,564 emergency room visits; 29,342 rural health clinic provider visits; and 995 school based clinic provider visits. Revenue and expenses related to these services are included above in box 4a.

Activity 3

The hospital also provided community health education and health screening services at reduced fees during the year in the form of a community health fair with 152 screenings; a board of education health fair with 25 screeenings; 192 sports physicals, school-based concussion testing and screening with 93 participants, diabetes education which served 1,104 participants and educational outreach to 192 students. Other activities include the heart walk with 92 participants, the nut run/walk with 103 participants, a pulmonary rehab open house with 20 participants, an employee wellness fair with 48 participants, and a dining with diabetes education event with 18 participants. Fitness complex memberships were provided for 565 community members.
