In march 2015, every voice center amended its articles of incorporation to change its legal name from public campaign to every voice center.

Form 990, Part VI, Section B, Line 11

The form 990 was prepared by the outside accountants and reviewed by the accounting consultant. A copy of the 990 was provided to each board member with additional documents for review and approval. If any changes were made, a final copy was also sent to the board.

Form 990, Part VI, Section B, Line 12C

Employees and the board are asked once a year at the december board meeting to complete their annual conflict of interest forms. To identify possible conflicts of interest, all every voice center staff, consultants, interns and volunteers must disclose to the president and ceo, or to the director of the washington national office, the existence of any financial interest in any entity with which that corporation or any legally related organization has or is negotiating a transaction or arrangement with every voice center and all material facts related to that interest. The definition of financial interest includes any direct or indirect relationship, through business, investment, or family, such as actual or potential ownership or investment interests or compensation arrangements. If every voice center staff, consultants, interns and volunteers violate this conflict of interest policy, the president and ceo, in order to protect the every voice center's best interest, take appropriate disciplinary action against the interested person. Such action may include formal reprimand, cancellation of the transaction or arrangement generating the conflict and/or suspension of employment.

Form 990, Part VI, Section B, Line 15A

All raises for the president/ceo, outside of cost of living adjustment, are initiated by the board of directors and are documented in the minutes of the meeting, based on current salary comparability data for nonprofit sector. The president determines compensation for all other staff. Aggregate staff compensation and cost of living adjustments are discussed with the board in the context of the annual budget process before making the increase. All final decisions regarding aggregate compensation are documented in the minutes of the meeting. In 2013 the compensation of all staff members was reviewed by an internal compensation committee which later reported to the board as part of the 2014 budget process. The committee used compensation information from other similar organizations and adjusted the organizations employees compensation accordingly in 2014.

The audited financial statements are also available through the charities registration bureaus in each of the states listed above. Financial Statement Notes

PART X, LINE 2:

For the year ended december 31, 2014, every voice center has documented its consideration of fasb asc 740-10, income taxes, that provides guidance for reporting uncertainty in income taxes and has determined that no material uncertain tax positions qualify for either recognition or disclosure in the financial statements. The federal form 990, return of organization exempt from income tax, is subject to examination by the internal revenue service, generally for three years after it is filed.

WE ALSO LINK THEIR CONCERNS WITH THE NATIONAL CONVERSATION ABOUT THE ISSUE AND INTRODUCE THEM TO NATIONAL POLICY ADVOCATES, MEDIA MAKERS, AND PROSPECTIVE FUNDERS. OUR RESEARCH AND ORGANIZING TEAMS DEVELOPED RELATIONSHIPS WITH GRASSROOTS REFORM PROPONENTS IN NUMEROUS STATES DURING 2014 AS WE CONDUCTED A MULTI-STATE INVESTIGATION TO DETERMINE GRASSROOTS INTEREST IN REFORM INITIATIVES.
