“Healthy harford is the healthy communities initiative of harford county, md. Founded in 1993 as a non-profit 501(c)(3), it is a coalition of local government agencies, businesses, non-profits, and citizens dedicated to improving the health of harford county residents through education, policy changes, improvements in the built environment, increased access to care, and improved care coordination. Promoting and supporting local resources, services, and businesses that support a healthy lifestyle is a large part of our vision to help make harford county the healthiest community in maryland.”

“Healthy Harford has a long history of integrating Social Determinants of Health as a means of improving chronic disease and behavioral health outcomes. This collaborative Collective Impact modeling is unique because of its close partnerships with governments, businesses, non-profit organizations, health care organizations, families, neighbors and friends. Through these cooperative relationships, Healthy Harford has successfully promoted and improved the health, strength and vitality of Harford County. All accomplishments achieved have be made possible through the power of partnerships. Highlights: -Secured a $10,000 from United Way. Identified the community partner, Harford County Office on Aging, to work with in the development, planning and implementation of the grant program. Identified the housing community (Fairbrooke Senior Housing in Aberdeen) for the program to be implemented. Invitation letters sent to residents announcing the program. Interviewed and selected a local healthy catering company that will be making and tailoring meals as well as delivering the meals to the participating individuals at Fairbrooke. At this time 5,000 dollars has been allocated to the Harford County Office on Aging as they are the partner that holds the contract with the caterer and will be providing oversite of the program. There are 18 residents who are participating in the program. The cost per week for each resident is $648 dollars per week for the 18 participants or about $36 per week per person. Featured Programs: -Walk with a Doc - At each Walk, we'll take a few minutes to learn about a health topic, then spend the rest of the hour enjoying a healthy walk at your own pace and distance. Held monthly at two locations. -Zumba Dance Classes - includes a variety of cardio, toning, and stretching exercises, as well as Spanish-bilingual classes. -Urban Line Dancing. -Maintains strong web and social media presence to ensure that residents are aware of resources and services available in the community.
