Financial Statement Notes

Part X : FIN48 Footnote

The Organization, pursuant to a determination letter from the Internal Revenue Service, is exempt from Federal and State income taxes under Section 501 (c)(3) of the Internal Revenue code and Section 23701(d) of the California Revenue and Taxation Code. Therefore, no provision for income taxes has been made. The Organization is not a private foundation.The accounting standard on accounting for uncertainty in income taxes addresses the determination of whether tax benefits claimed or expected to be claimed on a tax return should be recorded in the financial statements. Under that guidance, the Organization may recognize the tax benefit from an uncertain tax position only if it is more likely than not that the tax position will be sustained on examination by taxing authorities based on the technical merits of the position. Examples of tax positions include the tax-exempt status of the Organization and various positions related to the potential sources of unrelated business taxable income (UBIT). The tax benefits recognized in the financial statements from such a position are measured based on the largest benefit that has a greater than 50% likelihood of being realized upon ultimate settlement. There were no unrecognized tax benefits identified or recorded as liabilities at the current year ended.

IRS990/Desc0INFORMATION SERVICES & RESOURCES: From July 1, 2023, to June 30, 2024, Health Outreach Partners (HOP) conducted 25 webinar training sessions with approximately 3846 participants; over 57% are documented to be from community health centers or primary care associations. Health Outreach Partners collaborates with 20 National Training and Technical Assistance Partners (NTTAP) annually on webinars, conference workshops, trainings, and learning collaboratives. Participant numbers were collected for all our events, but health center status was not. Health Outreach Partners facilitated several in-person conference workshops, listening sessions, and trainings this year. HOP led and participated in 14 in-person events, 6 conference workshops, 3 training sessions, and 5 in-person listening sessions. These in-person conference workshops took place at the Western Forum for Migrant and Community Health, the Midwest Stream Forum, the East Coast Stream Forum, Community Health Institute, sponsored by the National Association of Community Health Centers, the Northwest Regional Primary Care Association Spring Summit, and the Idaho Community Health Center Associations Annual Outreach Conference. We did not present any virtual conference workshops this fiscal year. We had two in-person trainings, one on Trauma Informed Motivational Interviewing for Outreach Programs at the San Mateo County Department of Public Health, Healthcare for the Homeless/Farmworker Health Program and Outreach and Enrollment at Moses Lake Community Health Center, Quincey, Washington. In 2023, HOP committed to including the communities we serve in all program design, and the statement Nothing for us, without us representing our organizations values. Given this, we facilitated 5 in-person and 3 virtual listening sessions to inform future program design. The in-person events were held at the Western Forum for Migrant and Community Health, the Midwest Stream Forum, and the East Coast Stream Forum. In our various in-person events, we had over 355 participants, including health centers, community-based organizations, education, and leadership staff (including Board members). From April to July 2024, Health Outreach Partners (HOP) collaborated with HealthNet of Rock County, a free and charitable health center, to provide consultation and technical assistance in assessing financial and operational sustainability. HOP reviewed financial statements, available data, and staff interviews to complete our assessment and provide recommendations to HealthNet leadership for improved operations and funding diversification. Additionally, we offered a road map for applying for federally qualified health center status. Also, during this period, HOP led 12 Learning Collaborative sessions, all web-based interactive peer learning training sessions, which reached 183 unique individuals and 147 health centers. HOP authored four (4) publications and produced one health education podcast episode in this fiscal year for more than 1,213 downloads, and our resources, in total, received 42,546 page views combined. Among those publications were the Farmworker Health Network, Key Resource for Agricultural Worker Health, Outreach and Enrollment Training Curriculum for Health Center Staff and Outreach workers, the Organizational Self Care Assessment Tool Implementation Guide, and our Transportation is a Health Equity Issue fact sheet. We also produced a podcast episode discussing Long Covid to wrap up our series, The Covid-19 Pandemic and What It Taught Us: Collective Trauma and Moving Forward, episode 9, Rising Strong: Community Resilience in the Face of Long Covid, which has had 55 downloads.
