“Child and adolescent community services: care coordination (c&a cc) - alliance, inc.'s child and adolescent community services: care coordination (c&a cc) program works with youth experiencing various behavioral health diagnoses who are at risk of out of home placement. It is our goal to involve everyone who has a role in serving a youth with emotional or behavioral challenges in the process of supporting the youth and family. This includes families, advocates, educators, and mental health, social service, health service and addiction service workers. We use a strength-based perspective to allow a family driven experience with the goal of keeping youth in a family setting in their home and community while increasing access to community-based services. Youth who meet eligibility criteria will have access to intensive wraparound care coordination through alliance, inc. This process includes: offering strengths-focused assessment, building a child and family team led by the family, working with family to identify natural and community supports to participate on the team, giving families a voice and choice in creating and directing their plan, connecting families to community resources and making crisis support available from alliance, inc. Staff 24/7. Hours of operation are generally 8:30 am to 5:00 pm monday through friday with flexible scheduling as late as 8:00 pm if necessary. Enrollment is available in baltimore city, baltimore county, harford county and howard county. Number of clients served annually is 138. Individual supported housing (ish) - alliance's independent supportive housing program was implemented to develop long-term, affordable housing options for approximately 30 adult individuals with mental health disabilities within approximately 10 total properties spread amongst harford county, baltimore county, and baltimore city. Most commonly, tenants graduating from residential rehabilitation programs, or similar circumstance, would be potential tenants for this program. The program is structured as independent living and tenants would have previously demonstrated a level of independence while receiving more intensive supports. Tenants would have an array of community mental health support options available such as: outpatient mental health clinics for psychiatric and therapeutic appointments; psychiatric rehabilitation programs (prp) to provide on-site skills training, strategic goal development, and drop-in community and in-home support and intervention; vocational rehabilitation services to assist with job development, counseling, acquisition, and support; and comprehensive health home services. Tenants would be selected from local community-based mental health recovery programs. These individuals will have demonstrated progress in their mental health recovery and readiness for independence in the community. Number of tenants served annually is 3.”
