Green door housing was established to assist individuals with mental illnesses by providing supported independent housing to achieve independence and self-sufficiency. Green door housing ceased operations in 2018 following the sale of its assets.

Each year the board of trustees reviews a draft of the form 990. After review and discussion, the finance committee approves the form 990, including any adjustments needed. The 990 is finalized and the coo signs it and sends an electronic copy to each member before filing with the irs.

Form 990, Part VI, Section B, Line 12C

The green door housing conflict of interest policy (1) defines conflicts of interest: (2) identifies class of individuals within the organization covered by this policy: (3) facilitates disclosure of information that may help identify conflicts of interest and (4) specifies procedures to be followed in managing conflicts of interests. Board members are provided a list of green door housing vendors annually. Potential conflicts of interest are a consideration each time green door housing engages a new vendor. The board determines when a transaction may cause a conflict or potential conflict to occur. The policy provides that person with a conflict shall be recused from deliberations and decisions regarding the transaction.
