Form 990, Part VI, Section B, Line 12C

Each officer or director completes a questionaire to fully and completely disclose any conflict or potential conflicts of interest. The disclosure statement is completed upon his or her initial association with the organization and is updated annually thereafter. If there is reason to believe that an officer or director has failed to disclose an actual or potential conflict, the board informs the person of the basis for such belief and affords the person an opportunity to explain the alleged failure to disclose. If, after hearing the response and making such further investigation as may be warranted, the board determines that the officer/director has in fact failed to disclose an actual or possible conflict, it shall take appropriate disciplinary and corrective action.

Compensation has been based upon executive salary studies from both local and national trend data. Board leadership initiated a formal study, conducted by an independent contractor, last in 2005. The executive committee of the board meets annually with the ceo to conduct a performance review and to determine appropriate compensation and adjustments which are also based upon state increase criteria/stipulations, organization revenues, etc.

RESIDENTS ADMITTED INTO THESE PROGRAMS ARE LOCATED IN VR, JR AND FM COTTAGES, PLUS THE EXTENDED LIVING PROGRAM SUCCESSFUL STARTS.SERVICES SUPPLIED BY THE CHILDREN'S HOME INCLUDE HOUSING, TRANSPORTATION, DIETARY, AND PERSONAL CARE NEEDS. ALL RESIDENTS LIVE IN A HOME-LIKE RESIDENTIAL ENVIRONMENT COMPLETE WITH BED, BATH, STORAGE, AND LAUNDRY FACILITIES IN EACH COTTAGE. STAFF MEMBERS ASSIST RESIDENTS IN OBTAINING CLOTHING, SHOES, AND GROOMING SUPPLIES ALONG WITH SUPERVISION ON CARING FOR THEMSELVES AND THEIR BELONGINGS. PERSONAL HYGIENE IS MONITORED TO INSURE RESIDENTS LEARN THE BASIC EXPECTATIONS OF GROOMING.TREATMENT IS AVAILABLE FOR BEHAVIORAL OR TRAUMA-RELATED DISORDERS. SERVICES PROVIDED BY THE CHILDREN'S HOME INCLUDE MEDICATION ASSESSMENT, INDIVIDUAL PSYCHOTHERAPY, PSYCHIATRIC EVALUATION, GROUP PSYCHOTHERAPY, RECREATION THERAPY, MILIEU THERAPY, PSYCHOLOGICAL TESTING, AND FAMILY-CENTERED PROGRAMMING. ACADEMIC SERVICES INCLUDE SPECIAL EDUCATION, CLASSROOM INSTRUCTION, ACADEMIC TESTING, LIFE SKILLS PROGRAMS, SEX EDUCATION/STD/HIV INSTRUCTION, AND DRUG & ALCOHOL EDUCATION.MEALS ARE PROVIDED FOR THE RESIDENTS BY A PROFESSIONAL DIETARY STAFF FROM A COMMERCIAL-QUALITY, HEALTH DEPARTMENT CERTIFIED KITCHEN WITH CONSULTATION FROM A CERTIFIED DIETICIAN. THE DIETARY STAFF MEMBERS ARE SERVE SAFE CERTIFIED. STAFF MONITORS THE CHOICES EACH RESIDENT MAKES FOR NUTRITIONAL CONTENT. THE RESIDENTS HAVE AN AFTERNOON SNACK, GENERALLY A VARIETY OF ITEMS FROM FRUIT TO GRAHAM CRACKERS.OF PRIMARY IMPORTANCE TO THE PROGRAMS OF THE CHILDREN'S HOME IS EDUCATION. ALL RESIDENTS ARE ENCOURAGED AND EMPOWERED TO REACH A GOAL OF HIGH SCHOLASTIC ACHIEVEMENT. DAILY SCHOOL ATTENDANCE IS REQUIRED AND INCLUDED AS AN INDICATOR IN THE BEHAVIOR MODIFICATION SYSTEM. THE HOME PROVIDES A READING SPECIALIST WHO TESTS AND TUTORS RESIDENTS. EVERY COTTAGE, AS WELL AS THE ADMINISTRATION BUILDING, HAS A COMPUTER LAB. THE HOME OFFERS BASKETBALL, SOFTBALL, VOLLEYBALL, SWIMMING, HORSEBACK RIDING, AND DANCE. ALL SPORTS AND ACTIVITIES FOCUS ON FITNESS, SKILLS, TEAMWORK, AND GOOD SPORTSMANSHIP. ALL RESIDENTS ARE ENCOURAGED TO PARTICIPATE.THROUGHOUT THE YEAR THE HOME PROVIDES CULTURAL EXPERIENCES FOR ITS RESIDENTS TO BROADEN THEIR EXPOSURE TO DIFFERENT CULTURES AND IDEAS. CULTURAL PROGRAMS ARE PROVIDED ON-CAMPUS AND ON FIELD TRIPS THROUGHOUT THE MARYLAND / WASHINGTON DC AREA. THE RESIDENTS HAVE ACCESS TO ART PROGRAMS ESPECIALLY DESIGNED TO ENGAGE THEM IN PRODUCTIVE EXPRESSION, TEAMWORK, AND ENTREPRENEURSHIP.

OUT-OF-HOME PLACEMENT AFFECTING THIS POPULATION COMMANDS SPECIALIZED SERVICES AND THERAPEUTIC PSYCHIATRIC CARE. THE GOAL OF THE DIAGNOSTIC CENTER PROGRAM IS TO ASSESS, STABILIZE SYMPTOMS, AND RECOMMEND THE NEXT LEVEL OF CARE UPON DISCHARGE.THE DIAGNOSTIC CENTER PROVIDES STATEWIDE EMERGENCY PLACEMENT AND CRISIS INTERVENTION SERVICES TO ADOLESCENT GIRLS AGES 13-19, WITH THE MEDIAN AGE 14-16. TYPICALLY TEEN GIRLS ADMITTED ARE INFLUENCED BY THE EFFECTS OF LONG-TERM ABUSE, ABANDONMENT, AND/OR NEGLECT AFFECTING THEIR ABILITY TO FUNCTION EMOTIONALLY, COGNITIVELY, AND SOCIALLY. INITIAL ASSESSMENTS INDICATE COMMON BEHAVIORS RANGING FROM DEPRESSION, HIGH ANXIETY, SELF-DESTRUCTION, OPPOSITIONAL BEHAVIOR, INAPPROPRIATE SEXUAL BEHAVIORS, SUICIDAL THOUGHTS, AND DECREASE INTEREST IN ACADEMIC ACHIEVEMENT. OUR SERVICE APPROACH INCLUDES COGNITIVE BEHAVIORAL INTERVENTIONS WITH A FOCUS ON HOLISTIC HEALING AND STRENGTH-BASED INTERVENTIONS.SPECIAL PROGRAM EMPHASIS AND ADDITIONAL STAFF TRAINING ARE FOCUS ON TRAUMA-BASED CARE WITH THE INTENT OF ASSISTING RESIDENTS TO ACHIEVE TREATMENT GOALS WITHIN 90 DAYS. WITHIN THE DIAGNOSTIC CENTER THE TREATMENT PLAN STRIVES TO PROVIDE EVERY RESIDENT WITH A SAFE, HOME-LIKE ENVIRONMENT WHERE THEY GAIN PRO-SOCIAL BEHAVIORS, IDENTIFY COPING SKILLS, AND DEVELOP HEALTHY RESPECT FOR THEMSELVES AND OTHERS. EVERY RESIDENT'S TREATMENT PLAN INCLUDES THERAPEUTIC PSYCHOLOGY, SOCIAL AND RECREATIONAL THERAPY, ACTIVITIES OF DAILY LIVING, AND ACADEMIC ENRICHMENT.UPON ADMISSION A PSYCHO-SOCIAL ASSESSMENT IS COMPLETED. A PSYCHIATRIC EVALUATION IS ALSO COMPLETED TO DETERMINE THE LEVEL OF CARE AND SERVICES NEEDED TO HELP THE RESIDENT TO STABILIZE BEHAVIORS PLUS ADDRESS MENTAL HEALTH ISSUES.DAILY SCHOOL ATTENDANCE IS REQUIRED AND INCLUDED AS AN INDICATOR IN THE BEHAVIOR MODIFICATION SYSTEM. ON STAFF AT THE DIAGNOSTIC CENTER IS A CERTIFIED TEACHER. THE EDUCATION PROGRAM IS RECOGNIZED BY THE STATE OF MARYLAND AS AN APPROVED TYPE III SCHOOL WITH CLASSWORK IN SCIENCE, MATH, HISTORY, AND ENGLISH TAUGHT ON THE LEVEL OF THE INDIVIDUAL.RECOMMENDATIONS FOR THE FUTURE PLACEMENT OF DIAGNOSTIC CENTER RESIDENTS ARE MADE AFTER CAREFUL REVIEW OF RECORDS AND REPORTS FROM THE VARIOUS PROFESSIONAL STAFF THAT COMPRISES THE TREATMENT TEAM OF THE INDIVIDUAL.
