Form 990, Part VI, Section B, Line 12C

They are asked to disclose whether they are aware of any potential related party transaction or relationship in a questionnaire designed to help identify engagements that would result in a conflict of interest. In the event that a conflict of interest is identified, the respondent of the questionnaire is required to disclose the detail of the engagement resulting in such conflict, which will be reviewed by our independent compliance officer followed by a corrective course of action to eliminate identified conflict.

Form 990, Part VI, Section B, Line 15A
