The form 990 was prepared by an independent accounting firm based on the audited financial statements and information provided by management. The form 990 was reviewed by management and emailed to the board for review before filing.

Form 990, Part VI, Section B, Line 12C

San fernando community health center (sfchc) by-laws and policies include provisions that prohibit conflict of interest by members of the board of directors, employees, consultants, contractors, students, volunteers, and/or those who furnish goods and services to the health center. It is the policy of sfchc that all interests which could conceivably be considered to constitute a potential conflict shall be fully disclosed by any individual regardless of whether a conflict of interest is determined to exist. The disclosure shall take place for board members in the annual disclosure form and by employees through a conflict of interest acknowledgement form completed at the time of hire. Individuals are under continuing obligation to report any conflict or potential conflicts as they arise. Failure to comply with the conflict of interest policy will be investigated by the compliance officer and brought to the ceo and board of directors. Consequence for non-compliance with this policy can include: verbal/written discipline up to removal from employment or board of directors service.

DURING THIS FISCAL YEAR, SFCHC PROVIDED 27,002 PRIMARY CARE MEDICAL VISITS TO MEMBERS OF THE COMMUNITY. MAINTAINING OPTIMAL ORAL HEALTH IS AN IMPORTANT PART OF OUR DAILY LIVES. AT SFCHC, OUR DOCTORS OFFER COMPREHENSIVE DENTAL SERVICES TO ENSURE THAT PATIENTS IN AND AROUND SAN FERNANDO VALLEY, CALIFORNIA AND THE SURROUNDING AREAS HAVE LASTING HEALTHY TEETH AND GUMS. DURING THIS FISCAL YEAR, SFCHC PROVIDED 9,885 DENTAL VISITS TO THE COMMUNITY. SAN FERNANDO COMMUNITY HEALTH CENTER PROVIDES INTEGRATED BEHAVIORAL HEALTH SERVICES, SUPPORTING A 'WHOLE PERSON' MODEL OF CARE TO ADDRESS THE MIND/BODY CONNECTION. OUR MENTAL HEALTH PROVIDERS OFFER INDIVIDUAL AND GROUP CARE TO ADOLESCENT AND ADULT PATIENTS THROUGHOUT THE SAN FERNANDO VALLEY, LOS ANGELES AREA. DURING THIS FISCAL YEAR, SFCHC PROVIDED 6,013 MENTAL HEALTH VISITS TO MEMBERS OF THE COMMUNITY. SFCHC OFFERS NUMEROUS HEALTH AND WELLNESS EDUCATION RESOURCES, INCLUDING ON-SITE AND VIRTUAL COOKING CLASSES, SEXUAL HEALTH INFORMATION, NUTRITION CONSULTATION, AND REMOTE PATIENT MONITORING FOR CHRONIC HEALTH CONDITIONS.
