Tidalhealth nanticoke stands committed to providing that care. In 2023, tidalhealth nanticoke earned the joint commission's gold seal of approval for hospital accreditation by demonstrating continuous compliance with its performance standards. This gold seal is a symbol of quality that reflects a healthcare organization's commitment to providing safe and quality patient care and is valid for three years. In 2024, tidalhealth nanticoke was the only delaware hospital to be 5-star rated by the centers for medicare and medicaid services (cms) in hospital quality star ratings. Tidalhealth nanticoke is also one of just 381 hospitals in the united states of 3,000 surveyed, or roughly 8%, to earn this highest rating possible by cms after examining mortality, safety, readmissions, patient experience, and timely & effective care. For the first time in 2024, tidalhealth nanticoke was presented the healthgrades coronary interventional excellence award, placing that service in the top 10% in the nation. The seaford hospital was also 5-star rated for the first time in coronary intervention, treatment of heart attack, and treatment of respiratory failure. Tidalhealth nanticoke is also the only hospital in the state of delaware to achieve the healthgrades 5-star rating for the treatment of heart attack. Tidalhealth nanticoke participates in a proactive accountable care organization and continues to be recognized by the national council for quality assurance as a patient-centered medical home. Tidalhealth nanticoke also includes a cancer center that is accredited by the american college of surgeons commission on cancer. Our growth is evidenced not only through the continued addition to medical programs, but also through successful physician recruitment and expansion of our medical staff to over 32 different specialties. As our communities' healthcare needs change, tidalhealth nanticoke health services continues to change and expand services to meet those needs. Community outreach tidalhealth nanticoke's trained staff goes into the community to present health education seminars and provide health screenings. We work with area schools and community organizations to educate our community on disease management, care coordination, disease prevention and general wellness. There are many support groups and educational meetings hosted by tidalhealth nanticoke. Tidalhealth nanticoke works with the healthier sussex county task force and the sussex county health coalition on county wide initiatives. In addition, tidalhealth nanticoke has an active employee wellness program addressing health and wellness issues among its employed population. Community wellness tidalhealth operates a mobile, multidisciplinary community wellness team as part of the population health division of the system. This team of nurses and community health workers provide health screenings in underserved, low-income communities. Screening activities include review of social determinants of health, resource navigation, connection to pcp and health insurance. The team also provides blood pressure checks, diabetes risk assessments, and flu shots. Patients at high risk of hospitalization or readmission because of unmanaged sdoh or chronic conditions may be assigned a community health worker who conducts home visits to identify needs and connect the patient to community resources and health education to better self manage at home.
