Transforming age, a washington nonprofit corporation, is the sole member.

Form 990, Part VI, Section A, Line 7A

If not already serving on the board of directors, the chair and the vice-chair of transforming age's board of directors are ex-officio voting members of the board of directors. Transforming age's secretary and treasurer are also the secretary and treasurer, respectively, of this organization. Transforming age has the power to remove and replace any members of the board of directors.

Transforming age, as the sole member, has the right to (a) amend the articles of incorporation and bylaws; (b) adopt a plan of merger or consolidation; (c) authorize the sale, lease or exchange of substantially all of the organization's assets if not in the ordinary course of business; (d) authorize the organization's voluntary dissolution or revoke the proceedings thereof; (e) convert the organization into a for-profit corporation; and (f) adopt a plan for the distribution of the organization's assets.

The conflict of interest policy applies to all board members, officers and employees. Annually, a conflict of interest form is distributed to the board of directors and officers. The president & ceo reviews the completed forms and identifies whether any conflicts exist. Any actual or perceived conflict of interest of any governing board member or officer is disclosed to the other board members or highest level of management (president & ceo) and made a matter of record at the soonest possible time. Any governing board member having a possible conflict of interest may not vote or use personal influence on the matter, and is not counted in determining the quorum for the meeting, even where permitted by law. The minutes of the meeting reflect that disclosure was made, the abstention from voting, and the quorum.

The board uses an independent compensation consultant and compensation survey to annually determine the ceo's compensation. The ceo is responsible for conducting the review of other officers' compensation. Compensation was last reviewed in 2023.

Eastmont towers community holds security deposits on rentals/leases. The money is held in the organization's bank account. Security deposits are returned when the tenant vacates the property and the property passes an inspection.

PART V, LINE 4:

The wheeler memorial fund is an endowment established by a former resident and is used to support the eastmont foundation.

PART X, LINE 2:

Christian retirement homes is organized as a nebraska nonprofit corporation and has been recognized by the internal revenue service (irs) as exempt from federal and state income taxes under internal revenue code section 501(c)(3). Christian retirement homes is annually required to file a return of organization exempt from income tax (form 990) with the irs. In addition, christian retirement homes is subject to income tax on net income that is derived from business activities that are unrelated to its exempt purpose. Christian retirement homes believes it has support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. Christian retirement homes would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred.

THE APARTMENTS ARE KEPT UP TO DATE AND RENOVATED WITH QUALITY CONSTRUCTION. LIVING INDEPENDENTLY AT EASTMONT INCLUDES SERVICES SUCH AS A MEAL PLAN, TRANSPORTATION TO SHOPPING AND MEDICAL APPOINTMENTS, BI-WEEKLY HOUSEKEEPING, MAINTENANCE, AND A RICH PROGRAM AND EVENTS SCHEDULE. EASTMONT PROVIDES 24-HOUR EMERGENCY RESPONSE TO APARTMENTS AND RESIDENTS HAVE ACCESS MONDAY - FRIDAY TO A NURSE IN ONE OF TWO CLINICS ON CAMPUS. EASTMONT ALSO PROVIDES MANY OPPORTUNITIES FOR SPIRITUAL FULFILLMENT INCLUDING A CHAPLAIN WHO CONDUCTS SERVICES ON CAMPUS AND WILL ASSIST WITH SPIRITUAL NEEDS WHEN REQUESTED. THE HEALTH CARE TEAM WILL HELP RESIDENTS NAVIGATE DIFFERENT LEVELS OF CARE IF THEIR HEALTH NEEDS CHANGE.

RESIDENTS HAVE ACCESS TO LICENSED PROFESSIONAL NURSING STAFF ON CAMPUS 24 HOURS PER DAY. INDIVIDUALS HAVE TAILORED PROGRAMS THAT CAN INCLUDE ASSISTANCE WITH ACTIVITIES OF DAILY LIVING, MEDICATION MANAGEMENT, HEALTH MAINTENANCE ACTIVITIES, TRANSFERS OR AMBULATION, OXYGEN, AND ADDITIONAL COMPANION OR HOME HEALTH NEEDS.

THE HEALTH CENTER PROVIDES SKILLED NURSING, AND PHYSICAL, OCCUPATIONAL, AND SPEECH THERAPY IN AN INTERDISCIPLINARY ENVIRONMENT. ADDITIONALLY, SPIRITUAL GUIDANCE, SOCIAL WORK, CASE MANAGEMENT, AND CARE SERVICES ARE PROVIDED FOR THE OVERALL PSYCHOSOCIAL WELL-BEING OF THE PATIENTS. THE CARE CENTER IS GUIDED BY A MEDICAL DIRECTOR, LICENSED NURSING HOME ADMINISTRATOR, AND DIRECTOR OF NURSING.
