IRS990/Desc0Recuperative care, also referred to as medical respite care, is short-term residential care for individuals being discharged from the hospital without a safe place to continue healing and whose health conditions would be exacerbated by an unstable living environment. Recuperative Care facilitates a smooth transition from hospital care to stable housing by providing a supportive environment where individuals can continue their recovery and receive post-discharge treatment (i.e., medication management, therapy, etc.) while obtaining access to primary care, behavioral health services, case management and other supportive social services, such as transportation, food, and housing. Housing Navigation and Tenancy Services has three primary objectives: develop housing opportunities for people experiencing homelessness, support people in the process of accessing and moving into permanent housing, and provide supportive services to promote long term tenancy. Housing Navigation Specialists provide assistance with obtaining housing. Housing Navigation services include assistance with searching for housing or completing housing applications as well as developing an individual housing support plan. As part of the Housing Navigation service, clients receive assistance with housing security deposits, utilities set-up fees, first and last month's rent and first month of utilities. Members can also receive funding for medically necessary items like air conditioners, heaters, and hospital beds to ensure the new home is safe for move-in. Once housed, Housing Tenancy and Retention Specialists focus on assistance maintaining stable tenancy and may include interventions for behaviors that may jeopardize housing, such as late rental payment and services, to develop financial literacy skills.

Our goal is to serve all who are experiencing a housing crisis, including homelessness and housing insecurity, providing support, tools, and resources for them to move forward and achieve safety and stability. Our approach is to create a comprehensive continuum of housing services and resources that attends to the unique needs of every situation, from chronically homeless seniors to families experiencing homelessness. We are actively developing transitional and permanent housing projects while preparing to rapidly scale our services to meet the growing needs in our community.
