Prior to submitting the form 990 to the irs, it was reviewed by terry reilly health services' finance director and cfo, then by the foundation's board of directors.

Form 990, Part VI, Section B, Line 12C

Board members are required to annually complete a conflict of interest disclosure statement. If a board member has an actual or potential conflict of interest, they are required to disclose the material facts before addressing any transaction involving that relationship. After disclosing the conflict, the individual leaves the room for all further discussions and votes on the matter.
