CASE MANAGEMENT: THE CASE MANAGER PROVIDES DIRECT SERVICES TO ALL PATHWAYS CLIENTS WHO ARE IN-SHELTER. THIS INCLUDES THE CREATION OF GOALS AND WORKING TO COMPLETE THOSE GOALS, CONNECTING WITH LOCAL RESOURCES TO HELP THEM COMPLETE THEIR GOALS, PROVIDING TRANSPORTATION WHEN NECESSARY, AND FOLLOWING UP TO PREVENT CLIENTS FROM "FALLING THROUGH THE CRACKS".
