Governance Explanations

Form 990, Part VI, Section A, Line 2

The following officers and directors have a business relationship: keith goad, erik anderson, brian pace, willard hughes, lonnie, platt, and rob matuga.

Form 990, Part VI, Section A, Line 4

The organization amended its bylaws during august 2024 to allow the board of directors to elect a vice chairman or any other officer as it deems appropriate to enhance governance of the corporation. An additional amendment was made to allow the executive director to enter into any contract or to execute or deliver any instrument on behalf of the corporation.

Form 990, Part VI, Section B, Line 11B

The form 990 and its attached schedules have been completed by the executive officer, reviewed by the accountants, and at least one officer before submitting to the governance committee for approval, according to the board approved policy and disseminated to the board members and officers.

Form 990, Part VI, Section B, Line 12C

All board members execute an annual acknowledgement form affirming that each board member has received, reviewed and complied with the conflict of interest policy. Periodic reviews are conducted by the board to ensure that all jsi operations are consistent with its charitable mission and purpose. If an interested person fails to disclose a conflict of interest, the independent board members will immediately address the issue and take any necessary disciplinary or corrective action needed.

Form 990, Part VI, Section C, Line 19

Governing documents and financial statements are made available upon request and available to the public on the entity's website.
