QUESTION 6: HERITAGE VALLEY/UPMC MULTISPECIALTY GROUP, INC. HAS TWO MEMBERS, UPMC COMMUNITY MEDICINE, INC. AND HERITAGE VALLEY HEALTH SYSTEM, INC. BOTH MEMBERS ARE FEDERALLY TAX-EXEMPT ORGANIZATIONs UNDER SECTION 501(C)(3) OF THE INTERNAL REVENUE CODE. QUESTION 7A: THE MEMBER ENTITIES OF HERITAGE VALLEY/UPMC MULTISPECIALTY GROUP, INC., UPMC COMMUNITY MEDICINE, INC. AND HERITAGE VALLEY HEALTH SYSTEM, INC. HAVE THE ABILITY TO APPOINT BOARD MEMBERS TO HERITAGE VALLEY/UPMC MULTISPECIALTY GROUP, INC. AS STIPULATED IN THE BY-LAWS OF HERTIAGE VALLEY/UPMC MULTISPECIALTY GROUP, INC.

Entity board members, board committee members, and corporate officers-non-employed members of heritage valley/upmc multispecialty group, inc. Medical staff who hold a postion of influence of trust-individuals conducting clinical research at heritage valley/upmc multispecialty group, inc., whether or not they are employed by heritage valley/upmc multispecialty group, inc. These individuals are required to complete a questionnaire at least annually. The information, along with other data, is used to capture individual and institutional relationships so that potential conflicts of interest can be identified. If a potential conflict is identified regarding a specific heritage valley/upmc multispecialty group, inc. Activity, a conflict of interest subcommittee may be designated to review the information and report to the board of directors with results of the review and make recommendations to the board of directors for action or for further investigation or process. The process is ultimately overseen by a conflict of interest committee of the upmc board of directors on behalf of heritage valley/upmc multispecialty group, inc.

Heritage valley/upmc multispecialty group, inc. Ein: 47-3815667 fy ending june 30, 2024 heritage valley/upmc multispecialty group, inc. (multispecialty) was formed on april 10, 2015, as a pennsylvania state non-profit corporation and is formed in accordance with all requirements for tax exemption as a federally tax-exempt entity under irc 501(c)(3). Multispecialty is a joint venture between upmc community medicine, inc. And heritage valley health system. Both members are pennsylvania state non-profit federally tax-exempt entities under 501(c)(3) of the internal revenue code. The joint venture was formed in order to provide the community of the northern areas of pittsburgh, pa with the ability to receive quality neurological services in the convenience of their community. Multispecialty sees patients with all general neurological issues including seizure disorders, parkinson's disease, multiple sclerosis, neuropathy, radiculopathy, migraines, tremors and als. For the fiscal year ended june 30, 2024, the practice had 2,915 office visits, 1,123 consults, and did over 2,002 procedures including emg and ncv studies, eegs and readings, nerve blocks and spinal taps, and tysabri infusions. All patients are seen by multispecialty regardless of their ability to pay.
