THESE SERVICES, WHICH ARE NOT COVERED BY MEDICAID, ARE DESIGNED TO SUPPORT RECOVERY AND ENHANCE OVERALL WELL-BEING. MCHN ACCEPTS PATIENT REFERRALS FROM BOTH INPATIENT AND OUTPATIENT SOURCES ACROSS THE HEALTHCARE SYSTEM. AFTER SUCCESSFULLY SERVING ADULTS SINCE 2019, MCHN EXPANDED ITS SERVICES IN 2024 TO INCLUDE YOUTH REFERRALS. IN 2024, WE ACHIEVED: GROWTH IN PARTICIPATION AND ENGAGEMENT: IN 2024, 66% OF PARTICIPANTS (475 INDIVIDUALS) MET ALL OR SOME OF THEIR RECOVERY GOALS, COMPARED TO 60% (357 INDIVIDUALS) IN 2023. REDUCED INPATIENT ADMISSIONS AND ER VISITS DURING THE PROGRAM. SUSTAINED REDUCTIONS IN ER USE AND INPATIENT HOSPITALIZATIONS 6 MONTHS POST-PROGRAM. INDIVIDUALS SERVED ACROSS 29 COUNTIES IN MINNESOTA.
