We are a forward-thinking business that strives to provide our partners with valuable and innovative tools to improve healthcare and health outcomes in our region. The collaborative relationships and strategic partnerships we have formed with the partners and community stakeholders in our region has enabled successful implementation of transformational programs.
Flpps is a 501(c)(3) not-for-profit organization, and network of clinical and community-based provider organizations working together across 13 counties in the finger lakes region to transform the system of healthcare delivery for the medicaid population by bridging the gap between community and healthcare. We continue to drive transformation in the finger lakes region by ensuring the sustainability of successful programs and managing new, innovative efforts that bridge the gaps in the healthcare delivery system, including social, racial, and health inequity gaps that have been highlighted by the covid-19 pandemic. Flpps focuses on transforming healthcare with our partners under four strategic pillars: community programs, supporting partners, care management, and workforce.
