The form 990 is prepared by the external auditors and reviewed by management and the board of directors. Revisions, corrections, etc. Are made as necessary. Once all are satisfied with the form, it is finalized and a copy is sent to all board members and it is filed with the irs.

Form 990, Part VI, Section B, Line 12C

Each year the organization's conflict of interest policy is provided to all officers, directors and key employees. These people are asked to review the policy and sign a statement indicating that they understand the policy and have reported all potential conflicts during the past year in accordance with the policy and will report potential conflicts during the coming year. All potential conflicts are evaluated by the board to determine if a conflict actually exists. In those instances where the potential transaction is a conflict, the board examines the transaction and a vote is taken (with those involved recusing themselves) as to whether the organization will enter into the transaction.

The organization is exempt from income taxes as a public charity under section 501(c)(3) of the internal revenue code. Management believes that the organization operates in a manner consistent with their tax-exempt status at both the state and federal levels. The organization annually files irs form 990 - return of organization exempt from income tax, reporting various information that the irs uses to monitor the activities of tax-exempt entities. These tax returns are subject to review by the taxing authorities, generally for a period of three years after they were filed. The organization currently has no tax examinations in progress.

THE 4 M MODEL IS EVIDENCE-BASED FOR ADULTS AGED 65 AND UP AND ADDRESSES WHAT MATTERS TO THE OLDER ADULT, MENTATION, MEDICATION AND MOBILITY. THE QUALITY IMPROVEMENT PROGRAM INCLUDED TWO SUCCESSIVE COMPONENTS: AN ECHO LEARNING SERIES AND A QUALITY IMPROVEMENT INITIATIVE. OUR FOUR SESSION LEARNING SERIES "SUPPORTING PEOPLE LIVING WITH DEMENTIA-CLINICAL LINKAGES TO DELIVER DEMENTIA AND AGE FRIENDLY CARE" RAN IN THE SPRING OF 2024 AND INCLUDED 80 TOTAL PARTICIPANTS WITH SURVEY RESPONDENTS INDICATING HIGH SATISFACTION AND PRE/POST KNOWLEDGE INCREASING BY 26%. IN THE FALL, 2024, SIX PRACTICES PARTICIPATED IN "DELIVERING AGE FRIENDLY CARE: IMPLEMENTING THE 4 M MODEL AND SUPPORTING CARE PARTNERS OF PEOPLE WITH DEMENTIA: A PATH TO LEVEL 1 OR LEVEL 2 IHI AGE-FRIENDLY RECOGNITION" QUALITY IMPROVEMENT PROGRAM. PRACTICES RECEIVED AN INFRASTRUCTURE PAYMENT, PRACTICE FACILITATION SUPPORT AND PEER LEARNING SESSIONS WITH THE RESULT THAT ALL PRACTICES RECEIVED IHI AGE FRIENDLY RECOGNITION AND ALIGNMENT WITH THE 4 M GUIDELINES INCREASED BY 22% OVERALL (COMPARING PRE AND POST ADHERENCE RATES). RI MOMSPRN QUALITY IMPROVEMENT INITIATIVE:RI DEPARTMENT OF HEALTH CONTRACTED WITH THE CARE TRANSFORMATION COLLABORATIVE OF RHODE TO PROVIDE PROJECT MANAGEMENT AND QUALITY IMPROVEMENT SUPPORT FOR THE HRSA FUNDED RI MOMSPRN INITIATIVE. OVER THE COURSE OF THE GRANT, THE QUALITY IMPROVEMENT INITIATIVE PROVIDED SUPPORT FOR 15 PRACTICES THAT CARE FOR OVER 12,000 PREGNANT AND POSTPARTUM PATIENTS. THE PROGRAM INCLUDED SEVERAL ELEMENTS: PEER LEARNING COLLABORATIVE, ASSISTANCE WITH CAPTURING, REPORTING AND UNDERSTANDING AND ACTING ON DATA RELATED TO SCREENING FOR DEPRESSION, ANXIETY AND SUBSTANCE USE DISORDERS AND PRACTICE FACILITATION TO ASSIST WITH PROJECT MANAGEMENT, IMPLEMENTING QUALITY IMPROVEMENT CYCLES, AND DEVELOPING AND IMPLEMENTING IMPROVED WORKFLOWS. THE SECOND COMPONENT OF THE PROGRAM, A STATEWIDE CLINICAL TELECONSULTATION LINE, PROVIDES REAL-TIME SUPPORT FOR PRACTICES AND COMMUNITY-BASED ORGANIZATIONS IN RHODE ISLAND. THE TELECONSULTATION LINE PROVIDED ACCESS TO EXPERTS IN PRENATAL AND POSTPARTUM BEHAVIORAL HEALTH INCLUDING A SOCIAL WORKER, PSYCHOLOGIST AND PSYCHIATRIST WHO SPECIALIZE IN PRENATAL CARE. SCREENING RATES FOR DEPRESSION IMPROVED OVER THE THREE COHORTS BY CLOSE TO 30% (FROM 46.3% TO 66.1%). ANXIETY SCREENS INCREASED EVEN MORE, FROM 7.9% BASELINE TO 42.4% AND SCREENS FOR SUBSTANCE USE PRACTICES INCREASED BY 38% FROM 0.7% AT BASELINE TO 38.7% BY THE CONCLUSION OF THE PROGRAM. AS OF DECEMBER 2024, THE TELECONSULTATION LINE RESPONDED TO 3,054 CALLS FROM 1033 HEALTHCARE PROFESSIONALS AT 276 PRACTICES ACROSS THE STATE, ASSISTING 2,593 PREGNANT AND POSTPARTUM PATIENTS.
