“Organizations primary exempt purpose:polk county health and wellness is a nonprofit consortium of volunteers dedicated to making a difference in the health and wellness of the citizens of polk county, north carolina. Line 28 - do not lose hope/suicide remembrance walk:a program to bring awareness of and to reduce the stigma of suicide. The families of two young adults who died of suicide joined forces to start this annual program in polk county, nc. Once a year, a remembrance walk is held where dont lose hope flags are placed in a local park in remembrance of loved ones. In 2020, during our virtual event, 1,500 participants received a dont lose hope t-shirt. The virtual remembrance tribute reached over 1,000 views online. We also maintain a do not lose hope facebook page. Line 29 - hear2help:hear2help - a collaboration between polk fit fresh and friendly and foothills health network that provides and disseminates information on community health and wellness resources, as well as information about how to access these resources. The program porduces a community resource guide. Line 30 - community health & wellness:to provide members of polk county, nc education & opportunities to improve their health & wellness. Educational objectives are to: 1)provide lessons & information to the community to help those we care about be healthy & well; 2)provide mental health first aid training for employers, police offices, hospital staff, first responders & faith leaders; 3)promote mask wearing during the covid-19 pandemic to protect citizens of the community.opportunities to improve health & wellness in the community: 1)turkey trot eventa virtual 5k run or walk; 2)2020 virtual wellness challenge - teams of four commit to becoming more active over the 8-week challenge period; 3)the good life newsletter - create & distribute a monthly health & wellness newsletter.”
